Personnel & Service Truck(s)

Service Truck

  • Operator completed and documented a Pre-Trip Inspection of the truck

  • Service Truck is kept in clean & orderly to A-Core Standards

  • Service Truck has all required DOT items (Fire Extinguisher, Triangles, First Aid Kit, Strobe Lights, etc.)

General Work Environment & Housekeeping

  • Operator has completed a JHA, and communicated all hazards with other A-Core staff on site

  • Required Posters/Signs posted and legible

  • Measures are in place to protect the public or other trades from hazards created by A-Core

  • Operator communicated with others in the area that could be exposed to hazards created by A-Core

  • If needed, a SWWP was developed prior to work starting

  • Project has sufficient lighting to work and move around safely

Housekeeping

  • Work area is kept clean and clear of material and or debris

  • Slurry being created is vacuumed, funneled, and disposed of legally

Personal Protective Equipment

  • Hard Hats worn by all A-Core personnel on job

  • Eye & Face protection worn as required

  • Hearing Protection worn as required

  • Safety Toed foot protection worn

  • High Visibility shirt or vest being worn

  • Hand Protection worn as required

  • Respiratory Protection worn as required

Fall Protection

  • Job task requires elevated work above 6' from work surface or working near a leading edge

  • Operator have proper PPE required for fall protection

  • A Fall Rescue Plan has been documented and communicated

  • Fall protection PPE has been inspected prior to use

  • Tie off point able to withstand 5000 lbs. per person

  • Operator has a way to prevent tools and equipment from falling

Excavation, Trenching, and Shoring

  • Operator is working in an open excavation

  • Excavation is properly shored or sloped as per OSHA Regulations

  • Spoils, materials, and equipment set back a minimum of 2' from the edge of excavation

  • Egress is available in trenches 4' deep or more and 25' laterally from workers

  • Workers are protected from loose rock or soil

  • Trench boxes & Shoring have they been inspected and have proper pinning/bracing

  • Excavation is shored, sloped, or benched properly according to the soil type

Silica Dust

  • Operator is following the Table 1 chart

  • Integrated Water Delivery System is being utilized to eliminate silica dust exposure

  • Dust mask or respirators being used properly if needed

Electrical

  • All tools and power cords been tested and color coded as per A-Core's Assured Grounding Program

  • Ground fault circuit interrupters provided on all temporary electrical circuits

  • All extension cords have 3-prong plugs and all 3 prongs are intact

  • All electrical cords are in good working condition (no splices, frays, or the ground prong being removed, cord ends tight)

Hand and Power Tools

  • Operator is using the right tool for the job

  • Hand tools maintained and in safe working condition

  • Rotor Hammer Drills maintained in safe condition

  • Dustless attachment being utilized on Hammer Drill

  • Hammer Drill Bit being used is the correct diameter for anchor size

  • Hammer Drill Bit has legal side clearance for safe anchor setting

Flat Sawing

  • Operator Utilizing Diesel or Gas Powered Flat Saw

  • Flat Saw is maintained in a safe operating position

  • Flat Saw has been inspected prior to use (Greased, check oil levels, etc.)

  • Saw is being Operated in a well ventilated area without risk of carbon monoxide poisoning

  • All guards are in place to prevent injury

  • All oil levels have been checked and are at required levels

  • Operator utilizing 480V powered Flat Saw.

  • Flat Saw has been maintained in a safe operating position

  • Flat Saw has been inspected prior to use (Greased, electrical components, raise and lower switch, etc.)

  • All guards are in place to prevent injury

  • Saw is in safe operating condition

  • Integrated Water Delivery System is working properly

  • LOTO observed when changing/removing blade(s)

  • Proper blade RPM used for blade diameter per manufacturers recommendations

  • Operator is not wearing loose fitted clothing

  • Operator is deemed competent for task(s)

Traffic Control

  • Sawing Location does require Traffic Control

  • Proper signs placed on road to alert drivers of construction work ahead

  • The cones or barrels are set up to prevent drivers from entering work zone

  • Operator has the A-Core truck positioned to help protect himself and others

Underground Buried Utilities

  • Sawing location possess risk of utility strike

  • Utility locates have been called in prior to Saw Cutting Operations

  • Operator asked customer if we could potentially strike any utility

  • Sawing location was GPR Scanned prior

  • Customer signed field ticket prior to sawing operations
