General Work Environment

Fall Protection

  • Work being performed requires fall protection

  • Holes and other floor openings guarded by a grate, cover, guardrail, or equivalent on all sides (except at entrances to stairways or ladders)

  • Fall protection harnesses and lanyards used by every employee working 6 feet or higher above the ground

  • Fall protection harnesses and lanyards a minimum of 1/2 inch nylon or equivalent, and are short enough to prevent employees from falling more than 6 feet

  • Lifelines (lanyards) secure above the point of operation to an anchor point that will withstand 5,000 pounds of force for each attached worker

  • Harness, lanyards, and other fall protection equipment inspected prior to use

  • Where a job site has protruding steel rebar below the work level, the steel rebar protected by covers to prevent impalement injuries

Portable Ladders

  • Work being performed off of a portable ladder

  • Ladders are maintained in good condition and inspected before each use

  • Ladders being used for their designed purpose only (step ladders, A-Frame ladders, and extension ladders)

  • Employees are facing the ladder when climbing up and down

  • Employees standing below the top 2 steps when using the ladder

  • Ladders are used to gain access to elevated platforms, extend 3' above the elevated surface, and is secured properly

  • Ladder is placed on dry, stable footing, and properly secured

Baker Scaffolding

  • Work is being performed off scaffolding

  • Workers are utilizing Contractors or G.C's scaffolding

  • Scaffolds set up on level, stable ground with dry footings

  • Scaffold has been assembled and inspected by a competent person

  • Scaffolds higher than 10' have 42" high guardrails, mid rails half way, and toe board

  • Scaffold is in good working condition with no damage, worn, or missing parts

  • Planks on scaffold fit tightly with no open spaces between them

  • Open sides of scaffold are less than 14" from the work face

  • Guardrails are in place at the open ends of the scaffold

  • All pins installed where required and locked

  • Castors are locked during use

  • Castors or adjustment screws are pinned into the frame

Excavation, Trenching, and Shoring

  • Work is being performed in an open excavation or trench

  • Depth of excavation/trench work is being performed

  • Spoils, materials, and equipment are set back a minimum of 2' from the edge of excavation

  • Barriers are placed around excavation

  • Egress is available in trench within 25 lateral feet of workers

  • Workers are protected from loose rock or soil

  • Excavation is sloped or benched in accordance with soil type

  • Trench boxes inspected and in proper condition

  • Stacked trench boxes are pinned together

Electrical

  • All cords have been tested and are current with assured grounding program

  • Extension cords have 3-prong plugs and all 3 prongs intact

  • Electrical cords are in good working condition (i.e. no splices, frays, or the ground prong is removed)

  • Where electrical cords must cross roads, driveways, or walkways, are protected by solid, weather-resistant bridges to prevent damage to the cords and eliminate tripping hazards

  • Ground-fault circuit interrupters provided on all temporary electrical circuits

  • Electrical rooms/panels secured and have proper signage

  • Temporary lighting are equipped with bulb guards

Hand Tools - Jack Hammers, Chipping Guns, Rotor-Hammer, Etc.

  • Using the right tools for the job

  • Tools are inspected before use

  • Tools are maintained in safe condition

  • Power tools are current with assured grounding program

  • Grinders, saws, and other tools with moving parts, are equipped with appropriate safety guards to prevent physical contact with moving parts

  • Hand trucks and wheel barrows are in safe working condition

Saw Cutting Equipment

  • Demo crew is saw cutting or grinding concrete

  • Saw being used is in safe working condition
