Personnel & Service Truck

  • If needed, a SWWP has been developed prior to work starting

  • Project has sufficient lighting to work and move around safely

PPE, Excavations, and Fall Protection

PPE

  • Hard Hats worn by all A-Core personnel on job

  • Safety Toed foot protection worn

Excavation, Trenching, and Shoring

  • Is Operator working in an Open Excavation?

  • Excavation is properly shored or sloped as per OSHA Regulations

  • Spoils, materials, or equipment are set back a minimum of 2' from the edge of excavation

  • Trench has legal means of egress? 25' lateral feet from work area

  • Workers are protected from loose rock or soil

  • If trench boxes or shoring are used, they have been inspected and have proper pinning/bracing

  • Excavation is protected according to the soil type

Fall Protection

  • Does job task require elevated work above 6' from working surface or near a leading edge?

  • Operator is utilizing safe means to access work elevation

  • The tool or equipment being used to gain elevation been inspected prior to use

  • Operator is using proper Fall Protection

  • The Harness and Lanyard been inspected prior to use

  • Operator has a documented "Fall Rescue Plan"

  • The Tie Off point is able to hold a minimum of 5000 lbs per person

Housekeeping & Electrical

Jobsite Housekeeping

  • Work area is kept clean and clear of material or debris

  • Slurry being created is vacuumed, funneled, and disposed of legally

  • Cords & Hoses are strung or routed to prevent trip and fall hazards

  • Cords are strung or routed to prevent being cut, sliced, crushed, or damaged

Electrical

  • Tools and power cords have been tested and are color coded as per A-Core Assured Grounding Program

  • Ground fault circuit interrupters provided on all temporary electrical circuits

  • All extension cords have 3-prong plugs and all 3 prongs are intact

  • All electrical cords are inspected and in good working condition. No splices, frays, ground prong missing, cord ends are tight

Hand and Power Tools

Hand & Power Tool Safety

  • Operator is using the right tool for the job

  • All hand tools are maintained in safe working condition. Hammer, chisel, anchor setter, etc.

  • Rotor Hammer Drills are maintained in safe working condition

  • The Dustless attachment being utilized on Hammer Drill

  • The Hammer Drill Bit being used, is the right diameter for anchor size

  • The Hammer Drill Bit has legal side clearance for safe anchor setting

Wall Sawing

Wall Saw

  • Wall Saw is maintained in safe working condition

  • Proper blade diameter and blade RPM being used for depth of cut

  • Blade Guard is in safe working condition

  • Integrated Water Delivery System is working properly

  • LOTO observed when changing/removing blade(s)

  • Proper Anchoring is used for Wall Saw track feet

  • Operator is not wearing loose fitted clothing

  • Operator is deemed competent for tasks

  • Operator is using a 480V Powered Wall Saw

  • 480V Powered Wall Saw and Power Pack have been inspected and follow assured grounding program

  • Wall Saw Remote is in proper working condition

  • Cords and Leads are strung so to not be cut or damaged in anyway

  • Operator is using a Hydraulic Powered Wall Saw

  • Hydraulic Wall Saw been inspected (Hoses, Fittings, etc.)

  • Operator has kill switch or Deadman near Sawing operations

  • GPM is set to Manufactures recommendations

Hand Saw, Ring Saw, Chain Saw Tools

  • Operator is utilizing a Hand Saw

  • Saw has been inspected prior to use

  • Correct Power Source is used to power the saw (GPM, Electricity, Mixed Gasoline)

  • Proper blade is being used on saw

  • Operator deemed competent with a Hand Saw.

  • Operator is utilizing a Ring Saw

  • Saw has been inspected prior to use

  • Saw has been properly maintained (Greased, rollers are adjusted and within spec, etc.)

  • All guarding is in place & proper working condition

  • Correct Power Source is used to power the saw (GPM, Electricity, Mixed Gasoline)

  • Operator deemed competent with a Ring Saw

  • Operator is utilizing a Chain Saw

  • Saw has been inspected prior to use

  • Saw has been properly maintained and is in safe working condition

  • Bar and Chain inspected prior to use

  • Correct Power Source is used to power the saw (GPM, Electricity, Mixed Gasoline)

  • Operator is deemed competent with a Chain Saw

Silica and Traffic Control

Silica Dust Control

  • Operator following Table 1 chart for Silica Dust Controls

  • Integrated Water Delivery system being used and providing adequate water supply to the blade

  • If Sawing all day in enclosed area with no circulation, Operator is wearing a dust mask or Respirator

  • Controls are in place to prevent others from being exposed to Silica Dust

Traffic Control

  • Proper signs placed on road to alert drivers of construction danger ahead

  • Truck is placed in a way to help protect worker(s)
