COVID-19 Site Requirements

  • All contractors are required to develop for each job site a comprehensive COVID-19 exposure control, mitigation, and recovery plan with a Job Hazard Analysis (JHA). Is one Available for inspection?

  • Are workers educated in the language they understand best about coronavirus and how to prevent transmission and their employer’s COVID-19 policies?

  • Is the minimum six-foot social distancing between all employees (and customers) in all interactions at all times being observed?

  • Are other prevention measures such as the use of barriers, minimizing staff or customers in narrow or enclosed areas, staggering breaks, and work shifts, etc being used when social distancing isn't feasible?

  • Is personal protective equipment (PPE) such as gloves, goggles, face shields and face masks as appropriate or required being provided to employees for the activity being performed?

  • At a minimum, are cloth facial coverings being worn by every employee not working alone (with no chance of human interaction) on the jobsite/facility?

  • Are there adequate handwashing stations, with soap and running water, available throughout the jobsite/facility to allow frequent handwashing?

  • Has a housekeeping schedule that includes frequent cleaning and sanitizing with a particular emphasis on commonly touched surfaces been established

  • Are employees being screened for signs/symptoms of COVID-19 at the start of each shift?

  • Are the employees self screening prior to coming to work?

  • Has a COVID-19 Supervisor been identified for the site/facility to observe and enforce the COVID-19 Safety Plan?

  • Is the name and contact information for the site specific COVID-19 Supervisor clearly displayed on all jobsite COVID-19 required postings?

  • Is the COVID-19 Supervisor present on the job site/facility at all times whenever there are 7 or more employee present?

  • Are Safety Stand-Down/toolbox talk/tailgate training being conducted on all job sites on the first day of returning to work, and weekly thereafter, to explain the protective measures in place for all workers?

  • Are the meetings being documented and maintained for inspection?

  • Is social distancing being maintained at all gatherings (Stand Downs, Safety Meetings, Tailgate Meetings, Lunch and Breaks, etc.)?

  • Have “choke points” and “high-risk areas” on the jobsite/facility been identified and communicated to control those areas so social distancing is always maintained?

  • Are interactions being minimized when picking up or delivering equipment, products, or materials to ensure the minimum 6-foot social distance rule?

  • Are workers being encouraged/allowed to leave their workstations to wash their hands regularly, before and after going to the restroom, before and after eating and after coughing, sneezing or blowing their nose?

  • Is hand sanitizer, with greater than 60% ethanol or 70% isopropanol, available for use on the jobsite/facility as a back up for the soap and running water?

  • Are shared tools and equipment being sanitized between users?

  • Have policies been established which encourage workers to stay home or leave the worksite when feeling sick or when they have been in close contact with a confirmed positive case of COVID-19?

  • Have the appropriate disciplinary actions/procedures been established for employees refusing to wear the appropriate facial coverings?

Program Requirements, First Aid, and Housekeeping

  • Is the written APP tailored to hazards associated with work operations? 155-110(2)

  • Are crew leader safety meetings held at the beginning of job and weekly thereafter? 155-110(5)(a)

Fall Protection — Regardless of Height

  • Are stairways having 4 or more risers or rising more than 30 inches, whichever is less, equipped with at least one handrail and one stair rail system along each unprotected side or edge 155-477(3)(a)(i),(ii)

  • When there is a break in elevation of 19 inches or more, is a stairway, ramp or ladder provided? 155-476(1)

Ramps, Runways, Inclined Walkways

  • Is the ramp, runway or inclined walkway at least 18 inches wide? 155-24619(1)(a)

  • Is the ramp, runway or inclined walkway slanted less than 20 degrees on the horizontal surface and is it cleated or treated to prevent a slipping hazard on the walking surface? 155-24619(1)(b)

Dump Trucks

  • Do dump trucks used on site with an obstructed rear view have an operable automatic reverse alarm? 155-610(2)(f)

  • Is the alarm audible above the surrounding noise level and no less than 15 feet from the rear of the vehicle? 155-610(2)(f)(i)

  • If workers are in the backing zone or could reasonably be expected to enter the area, is there a designated backup spotter/observer OR a backup video camera for the driver? 155-610(2)(f)(i)

Cranes

  • Is the crane currently certified for the state of Washington? 155-52901
