Section 1: Workplace / Work Site Storage Conditions

  • (Workplace) The working conditions within these inspected workplaces are considered safe for employees to carry out their work tasks.

  • (Storage Outdoor) There are no stagnant water observed on any stored goods.

Section 2: Workplace Employees Safety, Health & Environmental Conditions

  • Site personnel / visitors are equipped with basic PPE such as safety helmet, safety shoes & other required PPE as stated in the main occupier's WSH in-house rules.

  • (Outdoor) Where workers are being exposed to working under extreme hot weather of more than 30mins continuous, workers are provided with drinking water and are well-hydrated.

  • There are clean and sheltered areas for workers to rest in.

  • (Workplace Toilets) There are suitable sanitary provided, maintained and kept clean, and equipped with sufficient and suitable lighting conveniences for the persons at work in the workplace.

  • The noise level in the worksite is consider acceptable.

  • Site personnel are resting in appointed rest area(s) only.

  • Individual site personnel at work are observed to have sufficient rest and not showing signs of fatigue.

Section 3: Fire Safety & First Aid

Firefighting services equipment

  • Adequate firefighting equipment provided in the workplace.

  • The firefighting equipment provided to the workplace has valid inspection records, ensuring its proper maintenance and functionality.

  • The firefighting equipment is never obstructed, ensuring unimpeded access and functionality at all times.

  • Emergency exit doors, corridors, and areas are maintained unobstructed at all times.

First-Aid

  • There is a sufficient number of first-aid kit(s) provided in the workplace and is maintained by the competent first aider(s) regularly.

  • In workplaces with over 25 employees, there must be first-aiders available during working hours at a ratio of one for every 100 employees or part thereof.

  • The first-aid items inside the provided kit are sufficient, adequate, and accurate, aligning with the first aid kit checklist.

  • First-aid items kept inside the first-aid box have not expired.

  • First aid kit is placed in an accessible area and displayed with first aid kit signage.

Section 4: Hand Tools / Electrical Tools

  • Hand tools / battery operated tools used in the workplace are in acceptable working condition state.

  • (Electrical) There are valid L.E.W (Licensed Electrical Worker) inspection records visibly displayed on electrical tools used on site.

  • (Electrical) Earth-leakage circuit breakers (ELCB) or Socket-outlet Assemblies (SOA) are used and deployed on the site for electrical safety.

  • (Electrical) The workplace's main temporary DB (Distribution Box) displays information including safety warning signs, details of the Competent Person-In-Charge, circuit drawings, and is secured with a lock.

  • (Electrical) All electrical tools in the workplace use industrial-grade plugs and socket-outlets when connecting to any electrical equipment in a temporary electrical installation, ensuring they are of heavy-duty industrial type.

  • (Electrical) Electrical / battery operated tools used are equipped with well-maintained safety guards.

  • (Electrical) Electrical cables used on site have no exposed 'live' wires, and all wiring is fully covered with the appropriate insulation material or electrical tape.

  • Lock Out Tag Out (LOTO) procedures are implemented in the workplace for the electrical system and/or machinery.

  • (Electrical) All the electrical cables / wires are either hung up above 1.8m or run along-side of the dry side of the wall to prevent tripping hazards.

Section 5: Roof Work Activities

  • There are sufficient guard-rails / barriers / parapet walls provided in the workplace to prevent any person from falling out from the WAH activities areas.

  • The installed guardrails provided have meet the requirement of implementing top guard-rail or the barrier of at least 1.2 meter above the work platform with 2 adjacent guard-rails provided does not exceed 600 millimetres.

  • There are travel restraint system lifeline being used in the workplace, is of good construction, sound material and adequate strength, free from patent defects and is suitable and safe for the purpose for which it is intended.

  • Travel restraint system lifeline is installed in accordance with the Professional Engineer's endorsed drawing.

  • Where workers are exposed to falling hazards, they are using the 100% tie-off method when moving from one place to another.

  • (Safety body harness) Where a fall arrest system is being used in a workplace, this system shall be in good construction, sound material, have adequate strength, is free from patent defects and is suitable and safe for the purpose for which it is intended.

  • Any loose tools / equipment / materials used near / at the edge of the building shall secured with lanyard attached to either the personnel's body or structure to prevent falling objects

  • (Roof) Where in a workplace any person carries out any work on any roof from which he is liable to fall off or through a distance of more than 2 metres, there are protection of the person against any sliding or falling from the roof (which may include crawler boards or roof brackets or sufficient and secured anchorage for the attachment of a full-body harness or a restraint belt).

  • (Fragile Surface) Where any person carrying out work at height may pass across or near, or work on, from or near, a fragile surface, there are prominent warning notices and full barriers affixed at the approach to the place where the fragile surface is situated.

  • Where ladder are being used near / at the edge of the building, a buddy system is being deployed to hold and stable the ladder and the ladder user is maintained 100% anchored to either P.E. endorsed anchorage point or lifeline provided.

Section 6: Working at Heights Activities

Ladder work activities

  • If hazardous work at heights, defined as work exceeding 3 meters in height, is being performed on ladders or near/exposed openings, a valid Work At Heights Permit-To-Work system must be in place for this inspection date.

  • Ladder kept or used on site are in good working condition.

  • Ladder user is equipped with the correct PPE (Safety helmet and anti-slip shoes) while working on the ladder.

  • (A-Frame) Ladder has cover to the top 2 steps to prevent ladder user from standing.

  • The ladder steps are in good condition with no signs of damaged / badly stains with chemicals / oily steps.

  • A minimum of one buddy system is in place to hold and stabilize the ladder while the ladder user is performing work.

  • Ladder user is maintaining 3-point contact while climbing up and down of the steps.

  • (A-platform) Ladder user has secure the ladder chain while standing and working on the platform of the ladder.

  • The ladder is positioned on a flat and stable surface, and there are no obstructions surrounding its use.

  • Ladder user secured loose hand tools with lanyard to prevent falling objects.

  • Ladder user is not overreaching while performing work on the ladder.

Scaffold Platform work activities (Tower / Mobile)

  • Are there any work performing on the scaffold platform work activities on-going on this inspection date?

  • Where work performing on the scaffold platform, there is a valid Work At Heights Permit-To-Work system applied for this inspection date.

  • Only competent scaffold erectors and scaffold supervisor(s) are responsible for construct, erect, install, re-position, alter, maintain, repair or dismantle certain scaffolds in the workplace.

  • The scaffold platform is constructed using materials in good condition with no rust to ensure its stability and safety.

  • A good condition and secure access ladder is available for users to safely reach and access the scaffold working platform.

  • A display indicating either "safe to use" or "unsafe to use" is attached as a valid tag to all of scaffold access ladder.

  • The scaffold working platform is equipped with an ample number of barricades and toe boards to ensure the safety of personnel working on it, preventing any risk of falling out.

  • The installed guardrails provided have meet the requirement of implementing top guard-rail or the barrier of at least 1.2 meter above the scaffold work platform with 2 adjacent guard-rails provided does not exceed 600 millimetres.

  • The scaffold platform is free from clutter, ensuring there are no tools, materials, or equipment obstructing the space.

  • (Mobile Scaffold) The mobile scaffold platform is equipped with a secure locking wheel mechanism designed to prevent excessive movement of the platform while users are working on it.

  • (Mobile Scaffold) While the mobile scaffold platform is in motion, there is no person present on the platform.

MEWP (Scissor / Boom Lift) work activities

  • A designated traffic warder is assigned to accompany and guide the movement of the Mobile Elevated Work Platform (MEWP).

  • Barricades are in place around the Mobile Elevated Work Platform (MEWP) to ensure no human beings are working beneath the work area.

  • Personnel working inside the Mobile Elevated Work Platform (MEWP) are equipped with fall arrest system such as safety body harness anchored to the MEWP designated anchor point.

  • Loose hand tools / materials / equipment used in the Mobile Elevated Work Platform (MEWP) are properly secured to prevent any potential falling objects hazards.

  • Operators operating the MEWP has completed the MEWP familiarization training conducted by attained MOM-Accredited general training center / supplier for the type of MEWP equipment.

Section 7: Lifting / Hoisting Activities

  • The surrounding of the lifting zone are fully surrounded with effective barriers with no signs of members of public accessing in and out.

  • Have the Lifting machine deployed fully all of its outriggers and is support with metal base plates? This applies to both mobile and lorry crane.

  • Are the lifting gears being used for lifting works have edge protectors?

  • Are there tags lines deployed for rigger to guide the suspended load?

  • Are competent riggers deployed to do rigging work for both mobile and lorry crane?

  • Lifting team members are maintaining full proper communication channels at all times. (Walkie -talkie / hand signal / Cell phones)

  • Lifting zone is kept cleared while load are being lifting up. This applies to mobile and lorry crane.

  • The weather is suitable for lifting / hoisting works.

Section 8: Welding / Hot Works Activities

  • Are there any welding / metal cutting works activities on-going on this inspection date?

Welding Works

  • Welding work is done at non-flammable areas.

  • Welder is equipped with welder's face mask, fire rated clothes & welder's hand gloves.

  • Gas cylinders are secured properly either inside holding cage or on the trolley.

  • Welding hose are in good condition.

  • Electrical welding set is properly grounded before use.

  • Electrical welding set has inspected and deem safe by L.E.W.

  • Electrical wires are either hung up above 1.8m or placed to the side wall tp prevent tripping hazards.

Metal cutting works / Gas cutting works

  • There are firefighting service (valid fire extinguisher & wet fire blanket) equipment available nearby with fire watchman on standby while cutting of metal works is on-going.

  • There are control measures implemented to contain the flying fire sparks.

  • The operator and the surrounding site personnel is equipped with ear plugs while work is on-going.

  • There are no combustible materials nearby while cutting work is being carried out.

  • Operator is equipped with protection face mask, fire rated clothes & hand gloves.

  • The cutting machine is equipped with safety guard and is inspected by LEW to deem safe to use.

Section 9: Chemicals / Hazardous Substances

  • There are valid Safety Data Sheet documents ob site and has been communicated to relevant stakeholders.

  • Chemicals / hazardous substances used in the workplace are kept in well-ventilated storage areas.

  • Workers handling the chemicals / hazardous substances put on PPE(s) recommended by SDS.

  • Chemicals / hazardous substances used in the workplace have proper labelling on the container(s).

  • Chemicals spillage kits available and place at accessible areas.

  • Proper ventilation equipment provided when workers are working with chemicals / hazardous substances used in the confined workplace.
