Is the "Work Authorisation" and "Permit to Work" (if required) available and does it cover the work being / to be carried out?

Has the site supervisor completed the "Reality Check"?

Have all personnel on site been inducted and signed the "Site Induction Record"?

Are the risk assessments and method statements adequate and accurate?<br>(If not, amend them and where necessary obtain a revised work authorisation / permit)

Are all personnel wearing their mandatory PPE?<br>(Head protection, hi-vis jacket / vest, gloves and safety boots with puncture resistant soles)

Are all personnel wearing their task specific PPE as required?

Are all operatives following the safe systems of work defined in the Construction Phase Plan?

Are all plant, equipment and scaffold inspection records available and up to date?

Is all the required third party protection and signage in place?

Is the site housekeeping and operatives appearance acceptable?
