Is the belt under Exclusive Control for Isolations? (To limit people working on the system to essential personnel only).

Is there an approved WIN for the task?

Have all essential people locked on to the Exclusive Control and all required isolation points?

Has the Authorisor on the Permit to Work (PTW) been informed of the upcoming belt movement and approved it?

Will the system be safe to work on from a 'working at heights' perspective if the belt is removed from the system?

If the belt is being removed from the system; Is the stringer height greater than 900mm to control a W@H hazard?

Are effective controls in place to manage any working at heights hazards created by removing the conveyor belt?

Is the belt pull calculation for the conveyor in the work pack and completed?

Has all chute liners/ hard skirt been removed from conveyor, escalate as line pull can be impacted.

Is all rigging and restraint equipment adequately rated, tagged and inspected to match the belt pull forces?

Are the correct pulling plates and clamp plates (to match belt pull forces) being used and correctly fastened to the belt?

Are all winches, winders, stands and turnovers restrained correctly for pull force calculated?

In line with the above question - Are counterweights sufficiently sized, restrained and positioned for restraint loads required?

Have winches and winders been pre-started and function tested for correct operation?

Is the take-up or GTU raised and secured correctly for the belt pull?

Have all scrapers and ploughs been backed-off or removed from the system?

Have all proxies, tacos and rip switches been removed or adjusted away from the belt?

Have hard skirts been checked for sufficient clearance of new belt and if required, raised or frames lowered?

Has the Belt been secured to the hub on the belt winder?

If using a pulling cable: Has the cable been attached to the pulling plate on the belt?

If using a pulling cable: Has the cable been attached to the hub on the belt winder?

If using a pulling cable: Has slack in the cable been taken up and tension applied before removal of the belt clamps?

Are the belt clamps removed from the system to allow belt pull?

Are the conveyor brakes disengaged to allow the belt to be pulled?

If pulling against direction of travel, has the backstop been removed?

Has the area around the winch, winder, stand or turnover been sufficiently barricaded to control hazards?

Are other work groups in the area aware of the belt pull commencing and any risks to them from product or water?

Has the Job Owner (Supervisor) delegated specific tasks for the operator of the belt winder(s) and spotters for the task?
