Boreholes

1) Is there a Method Statement on site specific to the activity?<br>i.e. Rotary, Shell and Auger

2) Are copies of relevant utilities plans on site?<br>a) Are operatives aware of overhead / buried services?

3) Has the Method Statement been briefed out and signed for by all personnel?

4) Are Risk Assessments available on site relevant to hazards that will be encountered?

5) Do operatives hold the relevant qualifications?<br>e.g. British Drilling Association card for foreman; CSCS for others.<br>a) What does the Method Statement require?

6) Have all operatives been inducted by the bore hole contractor and Principal Contractor?

7) Is the latest version of the Project Emergency Arrangements available on site?<br>a) Are operatives aware of contents and procedures?

8) What are the arrangements if a hazard is found during the operation? <br>e.g. voids.

9) Is there and inspection record for the plant?

10) Are copies of relevant lifting certificates available on site?<br>E.g. for wire rope and shackle on Shell and Auger, test cert for rig

11) Are COSHH assessments available on site for the substances to be used?<br>e.g. Bentonite, grease, oil

12) Is there an adequately stocked first aid kit on site?

13) Is there a qualified first aider on site?

14) Is there an in date, charged fire extinguisher with a pin inserted?<br>a) Is it stored in a secure manner that will avoid damage / accidental discharge?<br>b) Are fire hazards present?

15) Is there an in-date C.A.T with the team?<br>a) Has it been used prior to activities?

16) Is the rig situated on level and stable ground?

17) Is the working area free from tripping / slipping hazards?<br>a) Can operative move and carry items around the site safely?

18) Are all moving parts adequately guarded?

19) If compressed air hoses are being used are they adequately secured i.e. whip-checks or screwed fittings?

20) Are all hand tools in good condition and fit for purpose?

21) Are there any manual handling hazards which could be avoided?

22) Is adequate PPE available on site?<br>(Helmets, Eye Protection, Hearing Protection, Gloves, Boots, Overalls, Wet Weather Gear, Dust Masks)<br>a) What doe the Method Statement say will be worn?<br>b) Is noise a hazard from the rig?

23) Are the welfare arrangements adequate?<br>(Toilets, Hand Cleaning etc)

24) Can the vehicle and rig safely enter and exit the area?

25) Is the site left safe and secure at the end of the day><br>i.e. machinery locked off, boreholes safely capped.

26) Is there a process in place in the event of unforeseen / change in ground condition i.e. contaminated ground, running sand, high water table, mass concrete etc.
