  • The Dedicated Observer is positioned properly to observe the aerial work and can verbally warn employees of any encroachment issues.

  • Stop work, call an all-stop to ensure that the DO is properly positioned before proceeding

  • The Dedicated Observer is focused on work in the hot zone until the bucket is cradled (Cradle to cradle) Note: The dedicated observer can have no other duties while employees are aerial and working in the "hot work zone".

  • Was pole tested (probing, prodding, sounding) prior to completing any work including adjacent poles? Use N/A for all ground or maintenance work.

  • Stop work and have the crew test the pole and adjacent poles immediately.

  • If the line is de-energized, has it been properly tested, grounded and tagged properly?

  • If no, Stop work and contact the foreman's GF as this is a violation of Bowlin's LO/TO procedures

  • Was post job briefing completed for the previous days work? (100% required)

  • Address with the foreman this is required to close out the job for that day. The post job should include answers to the three questions, the time the live line was released, who they spoke to and a description if any special instruction was given by the DCC.

Personal Protective Equipment (PPE)

  • For BUCKET OPERATIONS, all PPE is being worn while completing work, including night work (hard hat, safety glasses, FR clothing). Enter in Notes if an at-risk behavior is observed

  • Who was not wearing proper PPE and what was the issue observed?

  • For GROUND OPERATIONS, all PPE is being worn while completing work, including night work (hard hat, safety glasses, work gloves, hi-vis safety vests). Enter in Notes if an at-risk behavior is observed

  • Who is the employee found in violation of the requirement?

  • Rubber gloves and sleeves are being worn for all line work?

  • Rubber gloves and sleeves have valid test dates and are being checked for deformities?

  • Are Work Gloves worn when handling materials?

  • Proper Work Clothing (no rips or tears in FR clothing including pants; work boots worn; and hi-vis clothing worn where required)?

  • Does the job require the use of a chainsaw?

General Electrical Work Requirements

  • The dielectric test dates are current for the vehicles assigned to the crew?

  • Call Eddie Jacobs immediately and schedule a dielectric test. If the truck has not been tested within 96 hours (4 days) of the violation, the truck will be shut down until a test can be completed.

  • Photo of the truck number is required.

  • Adequate and effective cover-up installed to protect the worker/equipment in the event of contact?

  • If a lack of cover is observed, call an all-stop to explain the issue and contact the GF

  • Photo of the cover issue is required to complete the inspection

  • Employee Secured in basket? (PFAS being worn at 6' or higher)

  • Is the employee climbing a utility pole?

  • If CLIMBING A POLE, has employee safely maintained three points of contact and maneuvered with Buckhoist/Supersqueeze/Rope Safety attached at all times?

  • Complete a Lineman Skills evaluation for the employee climbing

  • A hand line has been attached properly on the pole?

  • Materials are being raised/lowered with hand lines?

  • Call an all-stop and address the importance of having a hand/rescue line attached to the pole. The work is not to proceed until a the handline has been installed

  • Outrigger pads being used when outriggers are deployed?

  • Address the issue with the foreman. If the truck does not have outrigger pads, contact Brandon Adkins in the Warehouse, give him the truck number(s) and ensure he orders pads for the truck.

  • Employees are staying off of the bucket truck while the boom is in the "Hot work Zone"?

  • Call and all-stop and address the severity of this issue. The employee and foreman will receive at a minimum a verbal reprimand which should be documented using the iAuditor disciplinary action template.

  • Digger Derrick has been properly grounded?

  • Call and all-stop and ensure the digger is properly grounded

  • Digger Derrick ground wire has been completely rolled out (not coiled)?

8-1-1 Dig In Requirements

  • Does this job involve digging, soil penetration of any kind?

  • Have Utility locates been called in prior to starting work?

  • What types of utilities are within a 5 foot radius of the hole being dug? Check all that apply

  • What type of digging method is the crew using? Check all that apply

  • Has the foreman obtained positive response for the locate ticket?

  • Is the locate ticket valid? (Ticket completed within 10 days of the ticket request and completed within 21 days)

  • Have pre-dig photos been taken of the utility marks and/or flags?

  • If digging through concrete, are wet methods being used to prevent silica exposure?

  • Stop work immediately. Work is not to resume until locates can be completed

Workzone Protection

  • Does this job require traffic control of any kind (i.e flagger, closing a lane)?

  • Proper Workzone Warning Signage

Vehicles and Motorized Equipment

  • DOT Daily Log Books filled out for the truck(s) for that day?

  • Ensure Eddie Jacobs receives copies of the vehicle logs at the end of the week (Scanned, texted or given physical copy)

  • Pneumatic and Hydraulic hoses, winch line has been thoroughly inspected?

  • Loads Properly Secured?

  • Vehicle housekeeping is acceptable. (This includes the cab and bed of all work vehicles).

  • Add a photo of the housekeeping issue

  • What truck number is requiring immediate housekeeping?

  • Trucks have a Spill Kit, First Aid Kit and Fire Extinguisher?

  • Have employee/foreman contact Brandon Atkins for immediate replacements.

  • Warning/Hazard Lights Operable?

  • Vehicle chocks in use while parked? (Chocks placed properly on both wheels for uneven grade)

  • If repeat offender and employee has wheel chocks, a written warning will be required. If this is an issue that arises every visit, a suspension of 2 days will be assessed to foreman and employee assigned on the job briefing to complete the task. Complete the Bowlin Disciplinary form and have the employee sign the copy. Send copy to safety and Rick Ryan in HR.

  • Seat Belts worn by all occupants when moving? (Not wearing seat belt is CESR Event)

  • 1st offense- documented verbal warning using the iAuditor disciplinary form. 2nd offense- written and signed warning. 3rd offense- minimum of 3 days off using Bowlin disciplinary form. Send signed copy of the form to safety and Rick Ryan in HR

  • Spotter utilized when vehicle is in reverse? (Spotter required when two or more employees re available)

  • 360 degree walk around completed prior to moving?

  • Hands free communication is being used when mobile?

  • Document the employee who violated the requirement with a minimum of a verbal warning documented using the iAuditor disciplinary action template

  • If in motion, are all work vehicles leaving enough space between vehicles while in transit?
