1. Working at Heights

  • Have any falls greater than 6ft been addressed with the proper controls?

  • Have all unprotected roof edges or leading edges been marked and protected with proper controls?

Equipment - Lifting

  • Has any unsafe or damaged rigging been taken out of service and scheduled for repairs or maintenance?

  • Is all the lifting equipment obviously labelled, in accordance with OSHA requirements?

Equipment - Scaffolding

  • Is all the scaffolding complete and in good repair?

  • Are all employees aware of the Safe Work Load of the scaffolding?

  • Is there a scaffolding-competent person recognized onsite?

3. Hazardous Materials

  • Are all hazardous materials stored safely (e.g., flammables not near ignition sources, spill-able materials not near storm-water drains, etc.)?

  • Are incompatible chemicals stored separately from each other?

  • Are all hazardous materials stored in appropriately labeled and appropriate containers?

4. Electrical Hazards

  • Have any unsafe electrical leads (e.g., frayed cords, broken prongs on plugs) been repaired or removed from service?

  • Has any potential contact with underground utilities been addressed by professionals?

  • Has any potential contact with overhead lines been addressed by professionals?

  • Is all electrical equipment located away from hazards (e.g., water, flammables)?

  • Are all the employees familiar with the the Lock-out/Tag-out (LOTO) procedures for electrical equipment?

5. Traffic Management

  • Is there an adequate Traffic Management Plan (TMP) in place?

  • Are there adequate controls available for the TMP (e.g., physical barriers, bollards, speed limit signs, flashing lights, spotters, etc.)?

6. Work Environment

  • Are there adequate amenities (e.g., toilets, wash areas, lunch rooms, etc)?

  • Is there sufficient lighting for the work being preformed?

  • Is the trailer sufficiently stocked with equipment and supplies for the work being preformed?

7. Personal Protective Equipment (PPE)

  • Is the dust suppression system, if present, adequate for the environment?

  • Are the personnel on the job site adequately trained in the safe use, maintenance, and inspection of their PPE?

PPE - Individual Equipment

  • Do all the employees have adequate respirators/masks for their job?

  • Do all the employees have safe and adequate harnesses for their job?

8. Emergency Response

  • Is there an emergency plan at the site that has been communicated to all employees?

  • Are the first responder and emergency numbers posted onsite in an obvious location?

  • Are there adequate first aid kits and first aid equipment onsite, in obvious locations?

  • Is the route to the nearest clinic or hospital posted onsite in an obvious location?

9. Administrative Preperations

  • Is there a site specific safety plan developed for this jobsite?

  • Have all the permits been obtained (e.g., Council, EPA, WHA, Hot Works, Confined Spaces, etc.)?

  • Are all employees aware with the incident notification procedures?

  • Is there a trailer book available, complete with MSD sheets and weekly toolbox talk primers?

  • Do all current employees have the proper safety training and certifications for the current job?
