  • *** The approach by the observer is to demonstrate genuine concern for the worker’s health and safety. Observers should focus on good practices as well as identifying unsatisfactory practices. Observations should be short and take no more than 15 mins. Review past observations for the area of work, and previous actions. Use consultation and discussion to identify action and gain commitment from workers to change work habits. ***

POSITION OF PEOPLE

  • o Are pedestrians observing exclusion zones, 3m rule away from PME?<br>o Do walkways around PME movements have a solid barrier for separation?<br>o Can the operator access the PME safely without walking through a hazardous area?

PROCEDURES

  • o Procedures known and understood?<br>o Does the site have procedures for Forklifts and other PME? <br>o Does the operator require a licence to operate the type of PME being used?<br>o Are the keys removed from the PME when not in use?

WORK PRACTICES

  • o Are operators of PME aware of when pedestrians come into their area?<br>o Is the PME parked in an area that is easy and safe to access?<br>o Has a pre-start check been performed on the PME for that day?

HOUSEKEEPING

  • o Does the PME have rubbish around the seat or on the floor?<br>o If fitted with a windscreen is it clear and easy to see through?<br>o Does the PME run over paper or plastic that has the potential to be caught under the PME?

TOOLS & EQUIPMENT

  • o Are the tyres on PME in good working order?<br>o Is a seat belt fitted to the PME?<br>o Are all lights and flashing beacons working?

PERSONAL PROTECTIVE EQUIPMENT

  • o Check Eyes & Face, Ears, Respiratory, Hands, arms, Feet & legs, trunk (is equipment being used appropriate, is it fit for purpose and in good condition) <br>o Are electricians using electrically safe equipment and clothing?

    Note down any site, facility or building hazards noted during the observations may include; ( any broken equipment on the PME or building damage from PME contact) barricaded areas for contractors to work on PME safely.
