119.190 - SMS Requirements

  • Does the operator have an SMS that is appropriate to the size, nature and complexity of the operator's Australian transport operations? (119.190(1))

  • Does the Operator have an SMS that includes all FOUR PILLARS of an SMS (Policy and Objectives, Safety Risk Management, Safety Assurance, Safety Training and Promotion)?

  • Does the Operator have a Safety Policy and Objectives that includes management commitment to, and responsibility for safety; safety accountabilities of managers; appointment of safety management personnel; coordination of an ERP; SMS documentation?

  • Does the the Safety Risk Management Process include HAZID and safety RA and mitigation processes?

  • Does the Safety Assurance System include details of the processes for safety performance monitoring and measurement; management of change and continuous improvement of the SMS?

  • Does the Safety Training and Promotion System include the details for SMS training and education; SMS safety communication?

  • If regulation 119.195 applies (in relation to AOC) does the operator have a flight data analysis program? [Aeroplane > 27,000kg, Rotary > 7000kg or more than 9 seats]

Organisation and Personnel

  • Does the operator have a process it employs to regularly review and monitor the number of qualified personnel and decide whether additional staff are required to maintain safe operations? (SHOULD QUESTION) (GM119.110)

  • Are the roles , responsibilities and accountabilities of the key personnel outlined on the organisational chart and clearly articulated with respect of the operator's SMS? (GM119.110)

  • Is the CEO/Accountable Manager the person ultimately responsible for the safety of the operations and the SMS and that the Safety Manager has direct access to the CEO? (GM119.110)

  • Do managers ensure that sufficient resources are made available to achieve the outcomes of the SMS and to they continually monitor there areas of responsibility as laid out in the SMS Manual? (GM119.110)

  • If key personnel are absent and/or unable to carry out their responsibilities and it is likely/has continued for more than 35 days have CASA been advised? (GM119.110) AND does the exposition contain a description for how the organisation will manage the absence of key personnel? (GM119.110)

  • Do the FlyPelican key personnel responsibilities, qualifications and experience aligned the the AMC/GM both in the company manuals and in the actual persons assigned to these roles?

  • Does the Safety Manager regularly report to the CEO on the effectiveness of the SMS? (119.160(b))

Training and Checking for Critical Personnel

  • Does the operator provide training for key personnel to familiarise the personnel with their responsibilities and are details of the conduct and syllabus of this training in the manual? (GM 119.120)
