Are the 3 points of PPE being worn throughout site? (Hi-Vis top, Hard hat, Steel toe boots)

Are all job specific PPE being worn? (Dust masks, Rubber boots, Glasses, Gloves, etc...)

Are the right tools being used to complete the task?

Are tools being used correctly?

Are all checks completed?

Are ladders checked and tagged?

Are SPA's completed?

Are GA3's completed for ladders, harness's, nets etc....

Are Dig permits completed?

Are Hot works permits completed?

Are Toolbox talks completed?

Are all excavations stepped or sloped?

Are all excavations barriered off?

Have AF3's been completed for excavations

Is there safe access in and out of the excavation

Is there an advanced scaffolder on site?

Is the scaffold up to the code of practice standards?

Are all scaffolds handed over?

Are there a GA3's completed for all scaffolds?

Are gates kept closed?

Is tampering occurring?

Is all plant and equipment in good order?

Are all GA2's completed?

Do all operators have required training? (CSCS, IPAF etc...)

Is there exclusion zones around all plant? (Excavators, Cranes etc...)

Are there adequate fire extinguishers on site?

Are all Gas bottles stored correctly?

Is housekeeping up to standards?

Are welfare facilities clean?

Are internal trades wearing PPE?

Are internal trades cleaning houses when they are completed?

Who? Where?

Are walkways in place and to a good standard?

Is there adequate signage on site?

Where? What signs? Who is responsible?

Is there any vans on site?
