Observation Criteria

  • Has a JHA been developed for the task and readily accessible at the work face

  • Have all personnel engaged in the task signed onto the JHA

  • Has the JHA been reviewed and signed by the job supervisor

  • In the case of a lower tier subcontractor has the JHA been authorised by the principle contractor

  • Does the JHA identify Permit To Work requirements associated with the task

  • Does the JHA adequately reflect the task being performed

  • Does the JHA have an over riding Risk Assessment

  • Are the JHA task steps being followed

  • Have all potential hazards been identified (detail major hazards that have not been accounted for)

  • Does the JHA reflect any field modifications or other changes (traffic, environment etc.)
