Risk Assessment Checklist

  • This assessment is to be completed prior to commencing work. If you answer NO to any question you are NOT to start work until the issue has been resolved.

  • Signed into site and inducted?

  • Site Safety Plan and Safe Working Procedures are fully understood?

  • First Aider is readily identifiable?

  • First Aider details are:

  • Scaffolding and working platform are of the correct height and secure with safe access and egress?

  • Toe boards and brick guards in place?

  • Ladders are tied and at the correct angle of (1:4) and extends at least 1m above the platform?

  • Fascias are fixed correctly and access onto the roof is safe?

  • Rafters are braced and secured?

  • Crash decks, suitable air bags or Mansafe nets are in place?

  • Have any traps/openings been identified and suitably protected against falls?

  • Working area is free from overhead cables and obstructions?

  • Pre-use checks have been carried out on tools and items deemed safe for use?

  • Disc cutter has been inspected, safety checked and is fitted with a suitable and sufficient water dust supression kit?

  • Correct PPE is available ie. Hard Hat, Hi-Vis, safety boots, dust mask or RPE, goggles and gloves?

  • Materials can be safely hoisted into the working area (loading bay)?

  • Area underneath the working area is clear of other operatives/visitors etc?

  • Welfare facilities available and adequate?

  • Traffic routes known and suitable?

  • Signature of person carrying out assessment

  • Date/Time assessment carried out

Quality Assurance Checklist

  • Materials checked, correct and in good condition?

  • Work has been completed in accordance with the Contract Specification Sheet?

  • Site Manager/customer advised on completion?

  • Any outstanding issues apparent?

  • All debris has been cleared away to agreed areas for suitable disposal?

  • Any surplus materials identified to Contracts Manager?
