  • The Checklist is to be completed by the Master with the assistance of the Bridge Team. The Master is to be advised of any malfunction of equipment. Section 1 can be completed by the Master, Deck Officer or QM. Section 2 by the Master only. section 3 by the Master or Deck Officer. Boxes may be ticked or initiated, or makes N/A if not relevant to the ship. The master must sign section 2 and section 3 on completion of the checklist and before departure. The Deck Officer must sign Section 4 post departure and report all complete to the Master.
