Fatal Risks Do employees know and understand the three questions: 1. What are the fatal risks in my work place? 2. What are the critical controls keeping me safe? 3. How do I know those controls are in place and effecive? Can they answer those questions in regard to the monthly fatal risk theme? Are the fatal risks being identified and managed in personal risk assessments? JSAs? Are the Fatal Risk posters displayed? How about the site's bow-ties or registers?

  • Verification:<br>Confirm the site has rolled out the Fatal Risk presentation to all staff.<br>- the attendance record should have been submitted for updating of LMS

  • 'Gemba:<br>- Focus your plant/depot gemba on the fatal risk theme for the month<br>- use the presentation or site bow ties to identify causes<br>- use the presentation or site bow tie to identify controls

  • 'Verification:<br>- In your conversations with site employees, try to determine their understanding of the 3 fatal risk questions in relation to the monthly fatal risk theme

  • 'Verification:<br>- In your safety interactions with site employees, you should see evidence that the fatal risks have been considered in Take5s, JSAs and SAOs etc

  • 'Ensure site has access to their Fatal Risk register (bow-ties) and are familiar with using them

Critical Controls Has site completed their checlist? Is there an opportunity for you to participate in the completion of the monthly CCI checklist?

  • Verification:<br>Confirm that site has completed the CCI checklist for the month<br>- Have actions been raised in SHAERS for any non-compliance findings?<br>- Have actions from the critical control inspection been assigned appropriately? i.e. are there actions that require escalation?

  • 'If the checklist has not yet been completed, assist the Site risk Owner in completing it

  • 'Review a previous checklist and determine whether the actions (if any) raised have been completed and are effective

Safety Partners 'Preparation: Using the available resources, be familiar with the concept/message of this month's theme prior to visiting site Ensure you have your RAS cards with you on site

  • Confirm that site has the current Safety Partners focus poster displayed<br>- check that the site is including Safety Partners into site meetings

  • Use your interactions with site employees to understand their PB5 and encourage ways of making safety DIPI.<br>Try to understand the site's perception of how safety is prioritised

  • Leadership Influence<br>- Have you taken/created the opportunity to recognise safe behaviours on site?<br>- reflect on what messages (intended or otherwise) you have delivered while on site with respect to safety v production

  • 'Particpate in the completion of an internal audit checklist

  • 'Review the completed audit checklist(s) for the month.<br>- have actions been entered into SHAERS for each non-conformance?

Other

  • 'Review a selection of JSAs and confirm that they have been completed thoroughly<br>- are they describing all steps of the task?<br>- are they being signed by Supervisors?<br>- are team members signing on?<br><br>Could some of the JSAs being used on site be converted to a work instruction?

  • 'Select a work instruction in place for a site task and complete a SAO using the work instruction as a reference:<br>- Does the work instruction reflect the way the task is completed?<br>- are there improvement opportunities?

  • Consider using the Safe Act Observation Template on iAuditor

  • 'Check with Nadia and the site OHS Support Specialist to see if there any work instructions (new or old) due for review<br>- participate in a review of the work instruction<br>- assist by taking reference photos for the work instruction(s)

  • 'Through your interactions with site employees, provide and encourage the opportunity for site to escalate any issues to the WZHC

  • The key to a successful Gemba is to focus on process - when you have processes right, the results will take care of themselves.<br>Some safety processes for you to validate on your Gemba this month include:<br>- Risk assessments (Take 5! & JSAs)<br>- Communication (meetings, alerts etc)
