Exits and Aisles

  • Are all exits visible and unobstructed?

  • Can the fire alarm be clearly heard by all employees during normal operations?

Machinery and Equipment

  • Is all equipment clean and properly maintained?

  • Are all emergency stop buttons colored red?

  • Is lighting adequate for safe and comfortable performance of work?

  • Is there a power shut-off switch within reach of the operator's positions at each machine?

  • Is ventilation operating properly?

  • Are storage racks safe and undamaged?

  • Are storage racks adequate for the materials? Rated for the capacity?

  • If forklifts are in operation are preshift inspections being completed?

  • Has forklift operator training been completed and current?

  • If forklift being operated properly?

  • If scissor lift is in operation has operator received training and using properly?

  • If scaffold in use, has employee received scaffold competent person training?

  • If scaffold is in use, is scaffold constructed properly, used correctly?

  • If hoisting equipment or lifting straps are used has the equipment been inspected prior to use?

Personal Protective Equipment

  • Are employees trained on selection and use of PPE?

  • Is PPE training documented?

  • Is PPE being worn when required?

  • Is PPE easily attainable/available for employees?

  • Are the areas requiring PPE usage properly and clearly identified/posted?

Electrical Safety

  • Are electrical cords used in a manner to prevent damage from hanging on pipes, nails or hooks?

  • Are electrical cords free of damage, fraying, and exposed wire?

  • Are all panel boxes and disconnect covers kept closed?

  • Are all junction boxes, switch boxes and outlet boxes fully enclosed (no missing knock outs) and properly mounted?

  • Are all extension cords properly rated for hard/industrial usage?

  • Do extension cords have ground prongs?

  • Is there sufficient access and working space around electrical equipment?

Hazardous Substances

  • Do containers have complete and legible labels?

  • Are SDS's available for all hazardous substances?

  • Are liquids in closed containers?

  • Are food and beverages absent from production work areas?

  • Are proper chemical handling procedures being used?

  • Are chemical containers closed when not in use?

  • Are chemical containers grounded as necessary in dispensing areas?

  • Are compressed gas cylinders stored in upright position, secured as necessary?

  • Are compressed gas cylinders stored away from sources of ignition and substances such as flammable solvents and combustible waste?

Medical and First Aid

  • Are first aid supplies available?

  • Are first aid kits periodically inspected, filled?

  • Do all employees know how to get first aid assistance if necessary?

  • Is there a clear emergency response list posted for each work area?

  • Are eye wash stations in working condition, unobstructed and identified?

Housekeeping/Walking Working Surfaces

  • Are the working areas clean and orderly?

  • Are the floors clean and dry, spills cleaned up immediately?

  • Are the floors free from uneven surfaces or tripping hazards?

  • Are waste and recycling containers provided?

  • Are materials stored so they don't create a hazard?

  • Is fall protection provided for areas that expose employees to a potential fall of 4' or more?

  • Are ladders in use in this work area?

  • Are ladders inspected prior to use?

  • Are employees using the ladder correctly?

  • Are fixed ladders more than 24' in height outfitted with ladder climbing devices or cages? (Note, newly installed ladders more than 24' in height must have ladder climbing devices)

  • Any additional safety issues or comments?

  • Rate the overall condition of the workspace. 1 being the lowest, 10 being the highest.
