Mobile Equipment

  • Are inspections being completed on equipment, prior to being used before each shift

  • Mobile equipment license and training current within 3 years

  • Seatbelt used whenever forklift operated

Safety Shower / Eye Wash Station

  • Are all safety showers / eye wash stations operational

  • Are they inspected monthly

  • All safety showers are free from blockage - easily accessible

  • Are the caps present and closed to prevent dust collection?

  • Are emergency lights in working condition?

Emergency Exits/ First Aid

  • Aisles and emergency exits not blocked

  • Are exit signs illuminated and visible?

  • Are employees familiar with emergency response procedures?

  • Are 1st aid kits available and properly stocked?

  • Are emergency telephone numbers posted and up to date?


  • Is voltage indicated on electrical control enclosures, power cabinets,, junction boxes etc.

Material Storage Area

  • Material organized and rotated by date

  • Material not triple stacked, only two high

  • Are pallets in good condition

  • Are hazardous chemicals stored in appropriate areas?

  • Are flammable transfer containers connected to bonding cables?

  • Are flammable liquids kept in closed containers when not in use?

  • Are materials not stored within 18 inches of fire suppression sprinklers?

  • Is adequate lighting provided?

  • Are areas clean and free of debris?

  • Are floors free of oil spills or leaking material?

  • Is there adequate aisle space between containers to provide for container inspection?

  • Are tools stored in designated areas?

  • All other chemical storage hazards removed?

Flammable Cabinets

  • Only flammable liquids in cabinet (no paper, wood or plastic stored in cabinet)

  • Cabinets undamaged

  • No more than 3 cabinets within 100 square feet


  • All areas are free of spills

  • Are spill clean up materials available and properly stocked?

  • Are unprotected outdoor areas free of debris or potential sources of contamination?


  • Is there only one drum per satellite area

  • Is the waste pad organized

  • Do hazardous Waste have appropriate label (with date <90days)

  • Are the pallets with waste drums in good condition

Machine Guarding

  • Is point of operation (grinding wheel, etc.) protected such that inadvertent contact is prevented?

  • Flywheels, gears, shafts, pulleys, belts, gears, sprockets, chains and couplings guarded to protect employees

  • E-stop button present

Walking / Working surfaces

  • All holes or floor openings are guarded.

  • Are employees using hand rails while ascending/ descending stairs?

Fire Extinguishers

  • Monthly Inspection completed / Verify completely charged

  • Are Fire Extinguisher locations properly identified/visible ?

Confined space

  • Marked with proper signage (permit required sign or non permit required sign)


  • Are containers properly labeled. (Original container with Manufacturers Label or secondary container with HMIS label)

  • Do employees know how to access an MSDS for a chemicals used?

Lockout / Tagout

  • Are LOTO labels present

Compressed Air Nozzles

  • Are nozzles labeled as OSHA compliant. (regulate air pressure to 30psi or less)

Compressed Gas Cylinder Storage

  • Are cylinders identified with a durable label that clearly identifies the cylinder contents and gives a brief description of the potential hazard?

  • Cylinders secured with chain, lab strap or hard stock device located on the upper one third of the cylinder

  • Nesting methods for storage are prohibited. Acceptable means of storage include individual chains or straps, holding racks, cylinder carts etc.

  • Flammable gasses shall be stored at least 20 feet from oxidizers and any other source of ignition or separated by an approved firewall

Fall Protection

  • Are fall protection devices in good condition?

  • Are step relief devices in place?


  • Are tasks being performed with proper posture and body mechanics?

  • Are tools and work stations being used the best for proper body mechanics?

  • Are there any sharp edges at work stations that could cause nerve compression?

  • Are proper Lift techniques being used?


  • Are ladders free frome any loose steps, loose fasteners, missing, bent or broken steps?

  • Are the bases/shoes in good condition

  • Are the identification tags present and in good condition?

  • Are extension ladders in good condition? (no loose rungs, no corrosion, pulley free from fraying, etc)


  • Is proper ventilation being used in areas where chemical fumes are present?

  • Are chemical fume hoods on when needed and in good condition?

  • Are chemicals in fume hoods not stored in area for more than 1 day?


  • Are areas free of trip hazards?

  • Are areas free of clutter?

  • Has equipment been returned to storage area after use?

Combustible Dust

  • Are your combustible dust systems active with no faults?

  • Are your quarterly inspections completed?

  • Is there residual dust on horizontal planes/surfaces greater than the thickness of a paper clip?

  • The process is free of any noticeable materials leaks.

Hand Tools and Equipment

  • Are all tools and equipment maintained in good condition?

  • Are safety glasses, face shields, or other appropriate protective equipment used while using
    hand tools or equipment that might produce flying materials or be subject to breakage?

  • Are tools stored in dry, secure locations?
