  • This report is to be used by the assessor to inspect the site and record any safety, health and environmental hazards. The assessor should also pick an area of further inspection (e.g. SSOW, PUWER, Emergency arrangements etc) to ensure these are compliant with regulations and in- house procedures

  • Are details and arrangements for managing excavations detailed in the Project Management Plan?

  • Is the method statement specific in date and briefed out?

  • Have trained and competent persons been appointed to construct the excavation?

  • Have temporary works been agreed and relevant documentation been completed and signed for?

  • Does the excavation affect other parts of the project?

  • Have emergency response and evacuation been considered and highlighted?

  • Has a competent member of staff been engaged to inspect the excavation and safety measures in place?

  • Do the direct or indirect works to excavate fall under any other legislation or regulations?

  • Is there a defined no-go area around the excavation?

  • Are warning signs in place?

  • Is it effective?

  • Are the personnel responsible for inspection of excavations trained, qualified and experienced?

  • Have the correct tools and plant been selected for the excavation?

  • Have they been inspected and tested?

  • Is there a requirement for gas / air testing in or around the excavation?

  • Have locations of underground services been identified and briefed out prior to works commencing?

  • Has there been a consideration for contaminated land, i.e, surveys, reports, assessments?

  • Is the excavation protected from inadvertent vehicle risks (stop blocks, etc.)

  • Have support equipment been identities and checked for stability of the excavation?

  • Have considerations for dust / emissions been considered?

  • Please record the hazard rating using the following criteria for each occurrence:
    24 hour - score 5
    3 day - score 3
    7 day - score 1
    If items from previous inspection have not been closed out in allocated timescale immediately score additional 10 points
    This will give you a total hazard rating for the site.
    Next score the positive observations 1 point for each positive observation during the audit
    Where site demonstrates the implementation of observation cards add up to 4 points to this total - maximum points only if the cards are being used (more than 1 or 2 completed), the cards are being closed out, the site is displaying sorted=reported poster and details are being entered on EZONE
    For overall rating subtract the positive action total from the hazard rating total
