• This checklist is to be completed by the person proposing to enter a subfloor area beneath a house

Personnel training and PPE

• Have you had confined spaces training?

• Are you wearing suitable PPE?

• Do you have a spotter who has has had confined spaces training?

• Are there a reasonable number of air vents to allow cross-ventilation?

• Is your path clear of hazards such as broken glass or wire?

• Is there at least 500mm clearance

• Is the subfloor clear of large amounts of mould, sludge, or rotting vegetation?

• Will you spend less than 2 minutes in the sub-floor?

• 100% score - proceed with caution and watch for hazards and obstructions.
