  • The risk of a fatality from falling, dropped or flying objects increases when there are interactions within the drop zone.Engage with a working group (where there is a risk to others from uncontrolled, dropped or falling objects) and verify that there is clear separation / protection for all CMW's including barrier tape/barricading with information tag or signage. Spotter may be used instead of or in addition to.

  • The risk of a fatality from falling, dropped or flying objects increases when there are interactions within the drop zone or simultaneous operations are occurring in adjoining work environments.Identify an activity where simultaneous operations are occurring and there is a risk of multiple groups being impacted by dropped, falling or flying objects. Verify that a JSA/associated permits have been completed and the controls have been put in place and communicated. Engage with impacted groups and discuss what was discussed regarding simultaneous operations.

  • Has a JSA been signed off by all groups working in Simultaneous Operations where there is the risk of dropped, falling or flying objects?<br>If Simultaneous Operations underway :- Copy of Completed JSA & relevant permit is applicable - Signatures on JSA from crews working in simultaneous areas. - Verbal confirmation from impacted groups that simultaneous operations were discussed and controls communicated

  • Material build up introduces the risk of falling, dropped, flying objects. Choose 2 locations where there are walkways/roadways under conveyors (e.g. ROM or TLO). Verify that housekeeping is appropriate to prevent build up of material falling with the potential for a fatality.

  • Are house keeping standards sufficient to prevent substantial build up of material directly above walkways/roadways with the potential to fall and cause a fatality?

  • Take photo of work area
