  • Please review job pack and installation route. All penetrations into the building, through floors and walls need to be checked, marked and photographed prior to work commencing. Fixing penetrations for wall dogs, trunking, containment etc should be checked but do not need to be photographed and recorded in this sign off.

    - Broken or damaged hatches / doors
    - Rubbish / mess in risers
    - Broken or damaged paving slabs / bricks

  • Confirm ground will be scanned where digging / disturbance required?

  • Have you reviewed asbestos register?

  • Seek asbestos register from Project Manager

  • Are you confident that ACM will not be disturbed as part of installation works?

  • Do not proceed with any disturbance works. Seek guidance from Project Manager

Sign Off

  • We/I acknowledge the safe system of work provided and will work to the control measures outlined. Where the task deviates significantly from the Safe System provided we/I will raise with Project Manager to authorise.
