PRE OPERATIONAL FORKLIFT INSPECTION

  • NOTE:
    The accelerator, brakes, steering operation, drive control, tilt control, hoist and lowering control should always be operating smoothly. Once the machine is in operation and if a malfunction is identified, report immediately to your supervisor.

    In the event of a fault being revealed at any time or if the forklift truck becomes unsafe in any way:
    1. Report the matter immediately to your supervisor; and
    2. If the fault or condition is immediately dangerous, isolate and tag out and do not use the forklift truck until it is rectified.
    DO NOT TRY TO REPAIR THE FORKLIFT TRUCK YOURSELF

PRE OPERATIONAL CHECK

  • The lifting capacity is stated on the load data plate

  • The forklift is free from major physical damage (make note of dents, scratches and minor damage not noted on the previous inspection)

  • The machine is free of fluid leaks (check for patches of fluid under the machine where it has been parked)

  • Overhead guard, load backrest extension, seat belt and other safety devices are attached properly?

  • The tyres have the correct inflation and are free from damage

  • The lift and tilt systems including pins, chains and anchor points are free from damage

  • The heels of the forks free of cracks?

  • The forks are properly attached and the locking clips are in the proper position

  • The fluid levels for the brakes, cooking water, engine oil, transmission oil and hydraulic system are sufficient

  • All inspected hoses are undamaged and in good condition

  • Machine is fuelled/charged and ready to operate

  • Ensure the machine is aequately fuelled prior to use

  • The horn is operating

  • Are there any other issues worth noting

  • Issue details:

  • Has your inspection identified a risk that must be addressed before the machine can be used.

  • Provide a detailed justification for proceeding with using the machine below and contact management immediately to have the issue rectified.
