Formwork System Assessment

Have potential hazards associated with Formwork been identified and addressed in the Workplace Risk Assessment?

Are Formwork design drawings developed and signed by a Formwork designer or Engineer with copies available?

Has an Activity Method Statement been developed for the Formwork activity?

Is a Task Risk Assessment located at the work face that includes the formwork specific tasks?

Are the controls listed within the Task Risk Assessment being implemented on site?

Can the employees carrying out the Formwork tasks able to recite the hazards associated with Formwork?

Is there adequate safe access and egress to the form deck, catch platforms, temporary decks etc?

Has the leading edge been excluded and warning signs displayed to prevent unauthorized access?

Is the leading edge free from waste and excess materials?

Where applicable, are deck penetrations covered securely and appropriately signposted?

Is falling object protection installed on the perimeter of the Formwork deck?

Are exclusion zones and signs positioned below the form deck to prevent personnel entering the area?

Is there a process for the Formwork to be signed off by a competent person prior to pouring concrete?

Is the Formwork bracing positioned as per the Formwork design?

Is the Formwork stripping being conducted in a progressive manner?

Are the Formwork stripping areas barricaded to prevent personnel entering the area?
