  • Instructions: The purpose of this document is for the manager and the employee to discuss the employee’s performance.   • Attendance – reports to work when scheduled and on time.

  • Safety Compliance – follows safety practices in his/her work area; reports possible safety violations as appropriate.

  • Customer Service – is respectful to customers and visitors; acknowledges the customer in a timely manner; is respectful to all customers and visitors.

  • Responsiveness – answers all questions or obtains the answer for the customer and/or supervisor.

  • Productivity – accomplishes work tasks on a timely basis.

  • Quality of Work – monitors own work to make sure that there are not problems or errors.

  • Teamwork – helps co-workers with their tasks when needed; is respectful to all co-workers.

  • Follows Directions – responds favorably to instructions from managers; responds in a timely manner.

  • Work Appearance – wears Goodwill uniform or other appropriate clothing; clothes are neat and clean; wears appropriate shoes; exhibits clean hygiene.<br>

  • Any performance rating of 1 or 3 should have comments supporting the rating. Comments might include examples to support the rating and suggestions for improvement. The manager should identify corrective actions with the next level of management as well as the Director of Employee Relations.
