    Defect Reporting


    1.1 Where a defect or damage is identified on company vehicle, equipment and or machinery the operative is to report it to his Line Manager.


    1.2 The Operative is also responsible for recording the details of defects or damage in the Defect Book held in the stores.

    1.3 Should the vehicle/equipment/machine be found unfit to be used on health and safety grounds then the operative should stop using the vehicle/equipment/machine immediately. The Vehicle/equipment/machine should be clearly identified as unfit for use and Line Manager and Storeman informed.


    1.4 The Line Manager is responsible for arranging the repair of vehicle/equipment/machine. On satisfactory repair the Line manager shall sign off the defect book and remove any signage from the vehicle/equipment/machine identifying it as unfit for use. The vehicle/equipment/machine can now be used by the Operative.


    1.5 Where hired equipment is found to be defective then the supplier shall be contacted by the line Manager to arrange a repair.
