Administration & Documentation

  • Are fully insulated hand tools and a calibrated CAT available on site?

  • Has barriers/protection been installed around excavations to demarcate the area? Is it suitable for the depth and type of excavation?

  • Have excavations been supported to prevent collapse? i.e. battered back, stepped or excavation support system installed. Are stockpiles in a safe condition?

  • Has excavated material been placed a safe distance away from excavation banks?

  • Is there a suitable means of access into excavations?

  • Has exposed steel pins/rebar etc been protected from impalement using correct plastic caps?

  • Where reasonably practicable, are manholes being benched after the 1st ring has been installed to avoid confined space work?

  • If confined spaced access is required, has a permit been issued? Are are all persons involved competent? Minimum of two persons including a top man Is all necessary equipment in place (gas monitor, tripod & winch, full body harness & fall restraint lanyard, escape equipment) Is it certified and has it been inspected? Is air monitoring in place prior to entry and during entry?

Plant & Work Equipment

  • Is plant and work equipment in a suitable condition for safe use and operation? If there are faults/damage, has this been reported and recorded?

  • Is plant and work equipment being operated/used in the correct and safe manner?

  • Are lap belts being worn by all operators using mobile plant?

  • Has unattended plant been left in a safe condition? Have keys been removed from ignitions?

  • Have 'Thumbs Up' stickers been fitted to the rear of mobile plant where necessary?

  • COVID-19 - Is equipment that MUST be shared being cleaned before and after use?

Traffic Management

  • Has the Principal Contractor implemented suitable and sufficient traffic management arrangements throughout site?

  • Are pedestrian walkways in place to segregate plant/vehicles from pedestrians accessing working areas?

  • Are site speed limits being adhered to?

  • Have pedestrian segregation controls been altered or removed temporarily due to work activities? If so, have they been re-instated or have alternative controls been discussed and implemented?

  • Where work activities are conducted alongside haul roads, development access roads etc i.e. curb laying, remedial works, are exclusions areas set up to protect the workforce?

Emergency Arrangements

  • Are suitable emergency arrangements in place?

  • Have any accidents, incidents or near misses been reported? If so, has a incident/accident report from been completed and sent to Head Office?

  • COVID-19 - Is suitable PPE in place for first aiders (mask, eye protection & gloves)?

Welfare Facilities

  • Are welfare facilities compliant with Schedule 2, CDM 2015 regulations & site COVID-19 procedures?

  • Are welfare facilities clean, regularly sanitised, tidy and serviceable?

Protection of the Public & Third Parties

  • Are site boundaries protected from members of the public & third parties?

  • If boundary fencing has been taken down due to work activities, has the fencing been replaced if area left unattended or is a banksman in place to control the public/third parties?

  • Where work interferes with public areas, has suitable warning signage and suitable barriers been set out, maintained and effective in protecting the workforce and third parties?

  • Where work areas are being left unattended i.e. breaks, have all tools and equipment been left in a safe and considerate manner or removed from the area?

  • Have all openings (main and plot manholes, BT boxes, service openings, chambers, road gully etc) been protected with either the correct cover or as a temporary control suitable steel road plate, timber plyboard with temporary fencing? Not wooden pallets

Personal Protective Equipment

  • Is basic PPE being worn in compliance with site rules?

  • Is task specific PPE available and being worn as necessary i.e. impact goggles/glasses, hearing protection, fit tested respiratory protection (masks), gloves, wellington boots, coveralls etc?

  • Is PPE in a suitable and effective condition?

  • Is PPE specific for COVID-19 protection readily available, worn as necessary and sufficient stock held on site?

Occupational Health

  • Is manual handling being suitably controlled? Are mechanical devices being used where reasonably practicable?

  • Are dust suppression controls in place? Are they being used when cutting masonry?

  • Is exposure to hand arm vibration being reduced, controlled, monitored and recorded?

  • COVID-19 Protection - Is work being completed with 2.0m social distancing where it is possible to do so?

  • COVID-19 Protection - Where 2.0m social distancing is not possible are the agreed procedures being followed to protect individuals?

  • Are there suitable hand washing/sanitising facilities available at work areas or within a reasonable distance?

Housekeeping

  • Are working areas being maintained clean and tidy?

  • Are working areas being left in a suitable condition of others?

Environmental

  • Is there a designated refuelling area on site with a bunded diesel tank, drip trays, spill kits etc?

  • Is there any risk to nearby watercourses or live drainage from work activities? Are these areas being protected from contamination?

  • Are there any other environmental concerns that require action or attention?
