Section 1, Hazard Assessment

  • 1. Is the Tailboard/Hazard Assessment complete?<br> <br>

  • 2. Have the job hazards been identified, prioritized, and documented?

Section 2, Worksite Setup:

    Rubber goods shall be worn cradle to cradle when working within 6' of energized conductors or equipment.
    Sleeves shall be worn cradle to cradle when working within 6' of energized conductors or equipment.

  • Digger trucks have an insulated boom and should be grounded unless it is new construction

  • 17. Is line work being performed with the bucket in an effective ergonomic position?

Section 3, PPE and Tools:

  • 6 months on gloves, annually on blankets and hot sticks they are tested with the truck, ensure all leather gloves are free of holes or defects, check all possible PPE

  • 22. Is required PPE being used?

  • Includes safety glasses with side shields, hard hat, face shield, work gloves, hearing protection, as needed.

  • 23. When working on public streets, are reflective vests and/or flagmen in use?

  • 24. Are ladders in good condition with no visible defects, and are they being used properly?

  • 25. Are employees wearing FR clothing?

  • 26. If a grinder or saw is being used, is the guard in place?

  • This includes covers on chainsaws that are in the trucks

  • 27. Are overshoes being worn in appropriate areas/situations?

  • Overshoes should be worn when performing switching/grounding activities from the ground (distribution settings), mainly see these in Transmission section, 20kv and up with a barricade around the truck and wearing them inside the barricaded area.

  • 28. Is 100% fall protection being used when appropriate?

  • 30. Are cut-resistant gloves being worn when working with approved knives?

  • 31. Are tools being used properly and in good condition?

Section 4, STCTG:

  • 36. Are switching orders properly documented?

  • 37. Are switching orders executed in sequence, exactly as written?

  • 38. Have Danger Hold Tags been completed and installed as required?

  • 39. Are clearance orders properly documented?

  • 40. Are clearance orders executed as written, circuits tested to be de-energized, and grounds installed as required per task?

  • 41. Are MOS/gang operated switches/hookstick switches operated to fully open or closed position and properly disengaged and/or locked?

  • 42. Are breaker or control systems opened/closed/disabled/enabled/checked for correct operation and tagged as required?

  • 43. Are proper testing and grounding methods being used?

  • Check general condition of grounds and expiration dates

Section 5, General Inspection Items:

  • 44. Is the first aid kit and gel blanket present, stocked and all goods in date?

  • 45. Is housekeeping in vehicles and on the job site good?

  • 46. Does the employee have Safety Data Sheets for all hazardous chemicals present?

  • 47. Is the fire extinguisher charged, in good condition, and been inspected?

  • 48. Are seat belts in use when operating vehicles or equipment?

  • 49. Have all designated observers and craftsmen on site received the required training to be considered qualified?

  • 50. Does the employee properly use their cell phone and/or radio?

  • Use of cell phones, radios, and pagers is prohibited while hot-sticking or in the rubber glove zone unless in the event of an emergency. This includes the spotter. No cell phone usage while driving.

  • 51. Is the employee mindful of line-of-fire and pinch points?

  • 52. Are proper lifting and carrying techniques being utilized?

  • 53. Are proper pushing and pulling techniques being utilized?

  • 54. Are proper ascending/descending/climbing techniques being utilized?

  • This question includes the use of three points of contact, as well as safe and efficient bucket movement.

  • 55. Are all employees mindful of proper ergonomics/body placement for task?

  • 56. Are all linemen working within their "safe zone" (shoulder to waist)?

Vehicle/Trailer Inspection

  • 57. Do all headlights (bright and dim) and tail lights work?

  • 60. Do all turn signals work (left, right for front and back)?

  • 67. Are the mirrors clean and free of cracks?

  • 69. Are the tires in good condition (not excessively worn or flat, vehicle and or trailer)?
