SITE REVIEW CHECKLIST

  • 1. HazardCo Pack on site: There is a HazardCo Team Pack and resources readily available on site or there is a Site Pack containing a Site Specific Safety Plan for the job (large projects) on site, along with a hazard board?

  • 2. Public Protection: Signage is correctly in place and updated, fencing and barricades are in place and there is a safe pedestrian access past the site. There is a clear area available for loading and delivery and any construction areas are separated from any occupied areas of projects.

  • 3. Induction: All workers (including contractors) have been inducted to site using the job Induction Checklist & Register. Check that all workers have signed the induction Register. Has the client been given the Induction & Training canards and been formally inducted?

  • 4. Task Analysis: Does each contractor on site have their own Task Analysis for the work the are doing? Are they completing their work/tasks to the Task Analysis's specifications?

  • 5. Emergencies and Health: There is a trained first aider on site. Check that there is a first aid kit and fire extinguisher available on site. Check that there is access to a toilet and running water on site.

  • 6. Hazardous Substances: Check that hazardous substances are being controlled, a hazardous substance register is available and safety data sheets (SDS) are available if applicable.

  • 7. Personal Protective Equipment (PPE) : The appropriate PPE is being worn, is in good condition and used correctly. Head/face, hearing/eye, skin, footwear and protective clothing.

  • 8. Working at height: Appropriate fall protection is in place as per the working at height guideline. Protection such as scaffold, working platform, edge protection etc are in place and fall through protection eg mobile platforms, podium ladders, soft landing systems are also in place where required. If harnesses are being used, staff must be formally trained.

  • 9. Scaffolding: All scaffold is correctly braced and stabilized and at 3:1 height to base ratio. Scaffold is on firm foundations, not subsiding, plumb and level, base plates are in place and there is ladder access provided and used. A proper scaffold platform is provided (eg 3 planks wide or 675mm), planks are tied down, guard rails in place of 900 to 1100mm high with a mid-rail and two board or bottom-rail, within 300mm of the working face. Scaffold is tagged and certified if above 5 meters and is regularly inspected. Compliant with SARNZ best practice guidelines and manufacturers guidelines.

  • 10. Scaffolding over 5M: scaffold is tagged, certified, regularly inspected and has been notified to WorkSafe NZ.

  • 11. Ladders: There are no domestic ladders on site. Ladders are being used for short duration work (less than one hour) only, or as a means of access to a working platform. Letters in use are secured against movement at the top or footed, are in good condition, extend

  • 12. Electricity: There are tagged and tested RCDs in place. All leads are tagged and tested (3monthly) and are in good condition eg no temporary repairs or exposed wires. All are clear of walkways, high traffic areas and water.

  • 13. Falling Materials: Tools and materials are secured eg kept back from the edge and prevented from falling?

  • 14. Hand and Power tools: Staff have been trained in their use. Tools are in good condition and are being maintained, tagged and tested 3 monthly. Guards and isolation devices are in place where applicable, flying debris is controlled and appropriate PPE bring worn eg eye and ear protection.

  • 15. Noise: Noise levels are below 85 decibels or staff are wearing hearing protection?

  • 16. Excavations: Check that all the soil appear stable, there is no water accumulation in the excavation and existing services (electrical, sewer, gas, water) have been located and isolated (turned off). There are no objects or material stacked within 600mm from from the edge, trenching over 1.5m has been notified (to WorkSafe NZ) and shoring is in place. Excavations/Open trenches are listed on hazard board.

  • 17. Mobile Plant: (eg large excavators) The operator is protected by a roll cage, wearing a seat belt, working reverse signal alarms and high visibility clothing being worn. The existing services have been marked and pedestrians and visitors are kept clear.

  • 18. Gas Cutting and Welding: Appropriate PPE and shields are being used, fumes are controlled with ventilation, leads and equipment are in good condition and properly positioned/out of water. Blow-back valves are in place, cylinders are stored upright and secured and combustible materials are being controlled.

  • 19. Site Housekeeping: The site is clear of debris/rubbish, there are no trips hazards, there is clear access to work areas and waste bins are on site and being used.

  • 20. Site Deliveries: There is an adequate set-down are with suitable access to site, that is away from any pedestrian areas.
