Hazardous Waste Shipment Checklist

Uniform Hazardous Waste Manifest

  • Use a preprinted approved manifest form.

  • Provide site's Generator ID number (PRD987378437).

  • Provide emergency response number (i.e 800-424-9300).

  • Provide generator's site address;<br><br>PR-2 Km 60.3<br>Arecibo, PR 00688

  • Provide transporter's name and EPA ID number.

  • Provide designated facility's name and EPA ID number.

  • Mark (X) each hazardous material shipped.

  • Use proper DOT Description, refer to Site's Waste List.

  • Number, quantity, and type of containers match physical inventory.

  • Provide units and waste codes, refer to Site's Waste List.

  • Provide special handling instructions, as necessary; ERG#, Seal#, Profile#, Truck#, Chemtrect contract#, etc.

  • Provide the Land Disposal Restriction form.

  • Provide a continuation sheet for overseas shipment.

Containers/Package

  • Containers are in good conditions (i.e. no dents, no leaks, not corroded).

  • Containers are labelled; use appropriate hazardous waste label and appropriate DOT markings.

  • Containers are braced in the trailer, as necessary.

Trailer

  • Trailer is in good conditions.

Transporter

  • I certify that I have the emergency response guide for each of the material included in this shipment and being provided with the corresponding placards.
