Site Control/Supervision

  • Is there a nominated person in overall control of the site.

  • Are site contact names and phone numbers displayed?

  • Is a Safety Management Plan available (for projects over $250K)

  • Is there evidence of a daily toolbox talk

  • Do all persons on site have a Construction white card <br>(Directly supervised visitors or consultants exempt)

  • Do all Contractors on site hold a current HCC induction card?<br>(Directly supervised visitors or consultants) exempt)

  • Have all persons on site signed either the SWMS or Visitor Log?

Site Set-Up

  • Is worksite clearly delineated and signposted to identify work-site?

  • Are barriers adequate for site hazards?

  • Is there a Traffic Management Plan for site?

  • Does the set-up match the traffic plan?

  • Are Traffic Controllers qualified (sight evidence)

  • Are pedestrians directed safely around or through site?

  • Are all items adequately secured in event of wind or vandalism? (Signs, materials, barriers etc)

  • Are Site offices, toilets and containers placed in a way in which there is safe entry and exit (Door does not open into traffic or over gutters holes etc)?

Pre-Works Preparation

  • Has Dial-Before-You-Dig been performed?

  • Have necessary permits been obtained (Confined space, Hot work, Crane, Tasgas permit)?

  • Have services been located and marked?

  • Has site been checked for old town gas pipes using construction layer on Dehko Maps?

  • Has asbestos register been checked for presence of asbestos in buildings?

Hazard Identification & Controls

  • Is a JSEA or SWMS in use?

  • Has the JSEA or SWMS identified all of the hazards?

  • Have non-relevant hazards and controls been crossed off?

  • Are the safety controls detailed in the JSEA or SWMS being followed?

  • Are all fall hazards adequately controlled?

Electrical Hazards

  • Have low overhead powerlines been marked at ground level with bollards, tape and signs?

  • Have low overhead powerlines been isolated, diverted or covered with Tiger Tails?

  • Is there paperwork from Electricity supplier detailing hazard and controls and restrictions? <br>(VWD advice form)

  • Are all electrical tools tested and tagged?

  • Is there RCD protection for electrical tools? (Usually on Generator outlet plug panel)

  • Are extension leads in good order?

Personal Protective Equipment

  • Is appropriate PPE being used?

  • Is PPE maintained and stored correctly?

Plant & Equipment

  • Do all plant operators hold appropriate licences or competency?

  • Have Pre-Start Checks been conducted on plant?

  • Have faults discovered been reported and recorded as completed (check Fault Report book)

  • Are there any obvious defects on site plant not reported?

  • Has hired equipment been checked and is serviceable?

  • Are seat belts worn where required (If ROPS fitted)

  • Are all guards and protective devices in place?

Cranes and Lifting Equipment

  • Do cranes have any obvious unreported faults

  • Are Safe Working Loads displayed?

  • Do chain slings have current test dates labelled?

  • Are webbing slings free of oil and grease and cuts?<br>

  • Do webbing slings have Safe Work Load visible?

  • Are webbing slings stored in a way to minimise damage? (oil free, away from other tools, out of weather)?

  • Are hard hats worn in crane swing area?

Excavations

  • Is the SWMS for old town gas available and signed onto by crew?

  • Are safety instructions in DBYD observed (Safe distances, pot-holing,Stand-over)?

  • Are gas detectors worn when excavating by driver and crew close to excavation?

  • Are excavations benched, battered or shored if over 1.5 metre or there is a risk of collapse?

  • Is plant a safe distance from edge of excavation (1:1 depth to distance from edge)?

  • Are excavations barricaded to protect traffic and pedestrians where applicable?

Hazardous Substances

  • Are there SDS sheets available for all hazardous substances on site? (must be less than 5 years old)

  • Is there a Register of all substances? (In front of folder - Chemwatch Version)

Emergency

  • Is there an Emergency Plan for the site?

  • Is a team member first-aid trained?

  • Is there a suitable first aid kit on site and has it been checked in the last 6 months?

  • Is there a servicable fire extinguisher available?

  • Is there a suitable means of communication?

  • Is there suitable transport to medical help available?

Facilities

  • Are there suitable toilet and Hand washing facilities?

  • Are there suitable sheltered eating/rest areas?

  • Is there adequate clean drinking water available?

Smoking and Sun Safety

  • Is smoking controlled to minimise impact on other workers or public?

  • Are all persons adhering to HCC Sun-safe Policy?

Environment

  • Are erosion control measures in place?

  • Are there spill controls available? (dust, socks, absorbent pads)

  • Is silt or contaminated run-off prevented from entering drains?

  • Are dust/airborne debris controls in place if applicable?

Housekeeping

  • Is site tidy with no loose rubbish or trip hazards?
