Employees

PPE

  • Respiratory: protected for vapors, dust, misc. debris. Clean shaven, mask stored in sealed container

2.0 Ergonomics

  • Lifting/Lowering: weights not above 50# at/above shoulder height. Lift is not awkward or repetitive?

  • Pushing: versus pulling?

  • Reach: not overextended for long periods of time or during lifting?

  • Grip/force: not excessive when combined with high frequency?

  • Repetition: is not excessive not excessive when combined with high force and frequency?

  • Vibration: long exposures reduced by PPE, tape or tool design?

  • Body Posture: wrist, neck back in neutral position, elbows 90, close to body?

Equipment

  • Is the equipment in good, safe working order?

  • Are all machine guards I place and in use?

  • If equipment is equipped with backup alarms, lights, and horns; are they working?

  • Are vehicles equipment in need of , marking lights, strobe lights, warning signs for roadside safety?

  • Are all containers properly labeled? (SDS sheets available)<br>

  • Are ladders in good working condition and being used properly?<br>- belt buckle facing uprights<br>- not standing on top 2 steps<br>- 3 point climbing descending practiced

Vehicle

  • Is the vehicle parked in a safe legal spot?

  • Seat belts worn?

  • Safe Speeds followed?

  • Are the safety triangles/cones in use when parked for longer than 10-minutes?<br>

  • Are spotters used when backing vehicles?

  • Is the vehicle/equipment locked to prevent theft?<br>

  • Are the drivers operating the exiles in a safe and courteous manner?

  • Is the vehicle equipped and inspected and a working fire extinguisher onboard?

  • Is the cab free of trash and clutter?

Job Site

  • Are appropriate traffics warning signs (Men Working) posted in conspicuous places?

  • Have unsafe conditions that may cause injury or property damage been addressed?
