Slab Pour: Post-Tensioning

  • Block-outs for the mechanical ducts are correct

  • Slab is clean before pouring

  • Safety rails are in place correctly prior to pour

  • There are no cuts in the cables

  • Casings for cables are plumb

  • Rebar placement is correct

  • Imbeds at the stair well are correct

  • Shelf angle impeds on floors 3-7 is correct

  • Penetration sleeves for the electrical are correct

Columns

  • Rebar cages are tied at the bottom before lifting

  • Rebar placement is correct

  • Column forms are plumb

  • A qualified signalman is giving directions to the crane operator

  • Forms are correctly pinned

  • Safety straps are installed correctly towards interior

  • Sleeves for cables are installed correctly at 21" and 42"

Decks

Awning

  • Thru-bolt is in place

Safety/PPE:

  • Concrete finishers have rubber boots, gloves and safety glasses

  • Concrete vehicles are not interrupting regular traffic

  • Truck spotter is wearing safety vest

  • Flaggers with slow/stop paddles are available on site

Shoring Inspection

Inspect Shores and Reshores for the following defects, if any defects are found they must be replaced prior to the pour:

  • Cracked or broken material

  • Large knots

  • Vertical splits longer than 24"

  • Vertical splits which go from the edge of the 4 X 4 to an interior knot

  • Shores with more than 20% of the cross sectional area missing (less than 2-3/4" wide)

  • Broken Ellis clamps

  • Shores with holes

  • Shores with dry rot

Shores or Reshores not meeting the following requirements have not been properly installed. Deficiencies must be corrected prior to placement of the pour.

  • Shores are installed plumb.

  • The direction of every other shore bottom are alternating

  • Shores are located within 6" of the center of the standard mud pad, or at the center line of 2X_ mudsills.

  • Mudpads are placed on level, compacted fill.

  • Shores are not allowed to "scissor" at the clamp.

  • X-braces are located at a minimum of 16' on center, in each direction, and at the perimeter of the pour.

  • Shores are horizontally laced per the drawing requirements.

  • All shores over 14'-6" tall are single laced in both directions.

  • Re-shores at the perimeter of the building are laced to adjacent re-shores (1 row parallel to the edge of the slab, minimum) to prevent their falling out of the building.

  • No double decking (shores above shores), unless system has been engineered and approved by an engineer.

  • DESCRIPTION OF ITEMS REQUIRING CORRECTION:

  • ACCEPTED DEVIATIONS FROM SHORING AND RESHORING PLANS:

  • I have visually inspected the area described above and to the best of my knowledge, the shoring and reshoring is in general compliance with the shoring and reshoring drawings with the exception of any corrective work or deviations noted above. Once the above noted corrections have been completed, the shoring and reshoring for the above described area is released for the concrete pour.
