SP-3.2.11.1 Hazardous Substance Dangerous Goods Procedure

  • A risk assessment shall be undertaken for all HS/DG substances located on site<br><br>Site evidence that risk assessments are being completed by site employee for HS/DG.

  • A register will be kept and maintained for all HS/DG located on site. The register should be completed with information required on the register, the information is supplied on the Material Safety Data Sheet.<br><br>Site Evidence:<br><br>

  • Material Safety Data Sheets should be readily available so that Employees/Subcontractors can identify the substance, it's hazards and what precautions need to be taken.<br><br>site Evidence that:<br><br>-The MSDS represents the manufacturer or importers substance.<br><br>-The MSDS are in date no older than 5 years.<br><br>-Information in the MSDS is not altered.<br><br>The MSDS are available and kept near where the substance is being used.<br>

Non-conformance, CAR Reporting and Incident Investigation Procedure

SP-3.2.14.1 Management Procedure Preparedness Response Investigation.

Hazard/incident and injury Reporting

  • The incident reporting module is used for reporting incidents it captures incident reports and investigations and also notifies to Management of an incident.<br><br>Site evidence of the incident reporting module.

  • Investigations are undertaken to determine the Root Cause analysis, instigating future preventative measures.<br><br>Site evidence:

  • Incidents are communicated and risk reviews occur.<br>As a result of an incident the outcome of the investigations are assessed and the SWMS and Risk Register updated accordingly. Lessons learnt are shared with employees at Toolbox.

Incident reporting to worksafe

  • Project Managers and Health and safety reps are aware of the requirements for reporting and investigation and reporting deadlines to regulators.<br><br>Site discussion as evidence.

SP-3.2.01.1 Hazard Risk Management Procedure

  • Hard copy of the Project Risk Register is filed on site and available for all trades to access, this includes OHS & Environmental risks.

  • Design Risk Review is undertaken if changes occur to the design of the the works.

  • The subcontractor is required to provide a SWMS pertinent to their activities in support of risk and controls identified in the project risk register.

  • The Risk Register is reviewed monthly or sooner where changes are required as a result of site activities or other changes, the risk register must remain current against the activities throughout the life of the project. Where Changes or updates are made a CA&i report created by the PM, SS will provide instruction in relation to action to be taken by the site team such as review of relevant SWMS and toolbox meeting to inform relevant employees.

  • Part of the monthly reviews by the PM is to close out and archive relevant completed risk registers.

  • SWMS are to be reviewed and approve by the site supervisor or HSR prior to use on site using the SF-3.2.01.5 SWMS Verification form.<br><br>Site evidence of the verification of SWMS and where amendments may have been required.<br>Site evidence of 5 compliant SWMS in line with the verification register and approval.<br>

CA&I and Risk Modules

  • The CA&I module is used to record and manage the hazard identification process during the project. This includes OHS & Environmental Risks.<br><br>Site evidence that hazards and controls including root cause is occurring during the project.

  • CA&I reports are being closed out in a timely manor, and all relevant information and further actions are included.

SP-3.2.15.1 Subcontractor Management on site Procedure

  • Obtain subcontractor information subcontractor completes the checklist- SF-3.2.15.2 Subcontractor Pre-commencement Checklist<br><br>Site evidence for 3 newly appointed subcontractors.

  • All employees on site are to be inducted into the site prior to commencement of work, and a SWMS provided and and signed by all workers involved in the work.<br><br>Site evidence that all relevant documentation is completed in line with the SF-3.2.15.4 Induction Checklist, for the last 5 recent inductions.<br>Site evidence that all relevant employee qualifications are captured and that all documentation is signed and authorized by site representatives.

  • The site personnel log is to be available to all site personnel to sign in and out daily to keep track of who is on site in case of emergency.<br><br>Site evidence that all personnel on site have signed onto the log.

QP-4.2.01 Subcontractor selection Procedure.

  • Subcontractors shall be logged onto the subcontractor database. Performance of the subcontractor post projects shall be noted on the database- positive and negative feedback.<br><br>Site Evidence.

SP-3.2.10.1 Plant Management Procedure

Plant Induction

  • The plant operator or subcontractor is to complete the SF-3.2.10.1inspection report for vehicles and mobile plant, the report must be submitted for approval by B.E site management.<br><br>Site evidence for 3 items of plant.

  • Once a visual inspection of the Plant is conducted against the information provided in the inspection report, the plant is recorded in the SR-3.2.10.4 Mobile Major Plant Register, and all relevant information is captured on the register.<br><br>Site Evidence for 3 plant on the site.

  • On completion of work Plant listed in the mobile and Major plant register, is recorded date off site and all relevant documentation is filed in the project filing cabinet.<br><br>Site evidence.

Plant Risk Control Regulation 5.3.2

  • Risk Assessment must be conducted and risk control measures must be implemented.<br><br>Site Example where plant risk assessments are being provided by subcontractors.

  • A SWMS is to be provided for the woks to be carried out with plant on site, the SWMS should be submitted reviewed in consultation with employees impacted by the plant, the SWMS should be then approved by B.E site management or delegate, prior to work commencing.<br><br>Site Evidence of plant SWMS being reviewed by employees and approved by B.E site management or delegate.

  • A toolbox meeting should be held with all site personnel where high risk plant is coming to site to inform the of potential changes to the site layout, PPE requirements, and potential changes Emergency Evacuation routs or evacuation points.<br><br>Site evidence of consultation.<br>

Licensed Plant

  • Operators of certain types of Cranes, Hoists and Forklifts listed in Schedule 3 Part 2 and Part 3 must have a current High Risk work license (Occupational health and safety act 2004, section 41 and OH&S Regulations 2007, regulation 3.6.1)<br><br>Site examples of project documents that reflect licenses have been checked and recorded on Cranes, Hoists or Forklifts. record filed in subcontractor files in the filing cabinet.

  • Check all relevant plant licenses are sighted recorded in the QF-4.3.10 Subcontractor Employee Details form and other relevant documentation.

Plant Maintenance

  • An employer must ensure that measure to control plant related hazards are properly installed, used and maintained (OH&S Regulations 2007 regulation 2.1.1)<br><br>Site evidence maintenance records have been sighted of plant brought onto site by site management or delegate, Maintenance/Servicing is up to date and log book maintained daily.

  • Conduct weekly inspections of plant to ensure compliance to maintenance is to standard using the SF-3.2.10.2 Plant Weekly Inspection Checklist.<br><br>Site evidence of 3 items of plant.
