1.0 Training Program (Safety / Training Department)

2.0 Lock Out Tag Out Program (Safety / Maintenance)

  • Does the site have LOTO devices available to employees for the application of locking out a machine?

  • Does the site have a Tag system that may be utilized in the event a machine is not capable of being locked out? If no machines exist mark N/A. 1910.147(c)(2)(i)

  • Sites with tag systems have demonstrated that equivalent safety will be achieved as using a lock-out system? 1910.147(c)(3)(ii)

  • Where full de-energization is not practical, has the site demonstrated other means to provide full employee protection? 1910.147(c)(3)(ii)

3.0 Equipment Specifications / Safety Devices (Visual Inspection)

  • Are lock-out devices used for the sole purpose of controlling hazardous energy? 1910.147(c)(5)(ii)

  • Are all locks standardized across plant site? 1910.147(c)(5)(ii)(B)

  • Are all tag out devices substantial enough to prevent accidental removal?

  • Do tag out devices have warning label on them with identifiable labeling?

4.0 Employee Knowledge (Employee Questions)

  • Does the authorized employee properly utilize lock and tag devices?

  • Does the employee place a tag and lock on the energy isolating device?

  • Does the employee verify that the isolation and de-energization have been achieved? 1910.147(d)(6)

  • Each employee removes their device in accordance to plant policy?

  • Do Authorized employees understand all their responsibilities as it pertains to Locking out a machine?

  • Do employees recognize the dangers if a machine is not locked out properly and all procedures are not followed?

5.0 Documentation and Retention (Engineering / Maintenance / Safety)

  • Are training documents retained for the period of employment plus 3 years?

  • Have all retraining documents been maintained containing employees name and date of training? 1910.147(c)(7)(iv)

  • Have all annual inspections been retained for authorized employees?
