1.0 Training Program (Safety / Training Department)

  • When a case is deemed work related by workers comp and non-work related in regards to OSHA 1904, does the site maintain a Non Recordable Justification log with reason for not being counted on OSHA 300 log?

  • Are BLS safety surveys completed when required and on-time?

4.0 Documentation and Retention (Safety/Medical)

  • Does the site maintain an OSHA 300 log or equivalent?

  • Does the site post the OSHA 300A?

  • Does the site retain all injury reports readily accessible?

  • Are follow-up appointments for injured employees documented in some way by medical?

  • Are medical reports filed for length of employment plus 30 years?
