Emergency Equipment and HSE procedures (fire extinguishers, evacuation plans, JHA/JSAs, emergency contacts, etc)

  • Is the required OSHA Job Safety and Health Protection poster displayed in a prominent location?

  • Evacuation plans understood by all employees? Have they inquired with local management in the location they work?

  • Do fire alarms exist and are in working condition? Can all employees hear them?<br>

Housekeeping

  • Are areas organized and not cluttered (able to work without tripping or running into tools, material, etc)

Stairs and Ladders

  • Are all stairways at least 22 inches (55.88 cm) wide?

  • Do stairs angle no more than 50 degrees and no less than 30 degrees?

  • Are step risers of uniform height from top to bottom?

  • Are stairway handrails able to withstand loads of 200 lbs (90.7kg) applied within 2 inches (5.08 cm) of the top edge in and downward or outward direction?

  • Are surfaces elevated for than 30 inches (76.20 cm) have adequate handrails?

  • Are elevated surfaces equipped with toe-boards when machinery or work areas are exposed below.

  • Are all ladders maintained in good condition (joints attached, fittings secure, rails tight, etc)

  • Are all manufacturers labels clearly legible on the ladder?

  • Are nonslip cleats in good condition and not missing or broken on ladder feet?

  • Do portable metal ladders have CAUTION - Do not use around electrical equipment clearly displayed on them?

  • Are ladders being used properly and for their intended purposes (no standing on top steps, using ladder as a brace or platform, etc)

Personal protective equipment and fall protection

  • Is PPE provided by employer and is available to employee?

  • Are employees aware of how to properly use their PPE and adjust it?

  • Are employees selecting the proper PPE For the job they are doing?

  • Are employees wearing hard hats where the danger of falling objects exist?

  • Are employees wearing appropriate PPE at all times in the areas they are required to wear it?

  • Is food and drink being consumed in areas where no exposure to toxic chemicals, blood, or material exists?

  • Is industrial hygiene monitoring occurring at regular intervals (air quality, noise level, etc)

  • Are holes covered appropriately and able to withstand twice the heaviest load that is expected to go over them?

  • Are workers exposed to falls greater than 6 feet adequately protected with appropriate Fall Protection (guardrail system or harness)?

  • If in use, are harnesses and lanyards inspected before each use and monthly by HSE dept?

  • If in use, are employees trained in the proper use and maintenance of their harness system?

  • Are permitted and no permitted confined space procedures being utilized by employees when necessary?

  • Are approved respirators available when needed (see 29 CFR 1910.134 on respirators)? Do employees know how to use them and are fitted if necessary?

Rigging

  • Are operators moving loads above people? Does the operator know not to?

  • Are slings, wires, ropes and chains being used in good condition - no kinks, stretching, fraying, twisting or peening, etc.

  • Are operators using the load line or chain wrapped around the load instead of a proper sling? Are they lifting loads within rated limits?

  • Are hooks being used rated, in good condition, and the safety latch intact?

  • Are slings and chains being stored properly so they won't be damaged or become a tripping hazard?

  • Are hoist controls clearly marked to indicate direction of travel?

  • Are sling and chains being inspected and documented by the HSE dept on a regular basis?

Chemical Safety

  • Are approved containers and tanks used to store and handle flammable and combustible liquids.

  • Are NO SMOKING signs posted on liquified petroleum gas tanks and in areas where combustible or flammable materials are being used and stored?

  • Are employees aware of potential hazards and trained in safe handling practices for various chemicals stored or used in the workplace?

  • Are all containers labeled properly with their contents and hazard rating, if necessary?

  • Are waste chemicals stored properly?

  • Are containers and aerosols covered and capped properly?

  • Are chemicals segregated properly? Are powders above liquids?

  • Are flammable/chemical cabinets in good condition?

  • Are chemicals approved by HSE before brought onto work site?

  • Do employees use appropriate gloves, aprons, and face shields when handling chemicals?

  • Is there an adequate ventilation or dilution system where chemicals are being applied or stored?

  • Do employees show signs on skin or affects (dizziness, red eyes, etc) from chemical exposure? Ask.

  • Are SDS/MSDS available to employees at the work location and available anytime?

  • Are there adequate spill control measures in place (secondary containment, spill kit, etc)?

Electrical safety

  • Are extension and power cords properly stored?

  • Are extension cords in good condition (no frays, haphazard repairs or exposed wires - insulation intact)?

  • Are electrical panels labeled appropriately with voltage rating, danger signs, etc?

  • Are all electrical raceways, equipments and enclosures securely fastened in place?

  • Are all electrical enclosures such as switches, receptacles, junction boxes, etc., provided with tight fitting covers and plates?

  • Do all employees know where the disconnect switches for the equipment they are working on is located?

  • Are all plugs, sockets and switches complete without cracks, breaks, exposed wires?

  • Is there sufficient access space to work in and around electrical equipment (min 36 inches) safely?

  • Are circuits overloaded or show signs of overloading or shorts (burn marks)?

  • Do employees know proper lock out- tag out procedures? Do they do the tags correctly?

  • Are there any liquids submerging electrical cords or leaking/running in or on electrical equipments?

Equipment (lifts, jacks, hoists, forklifts, etc)

  • Are hand carts and lifting jacks in good condition? Are they being used properly?

  • Are hoists being inspected before use by the operator and on a regular basis by the HSE dept and documented?

  • Are power cutoff switches accessible to the operator in case of an emergency?1

  • Is the crane load rating visible and legible to the operator from the ground?

  • Are hoist limiters and safety switches in place and functioning correctly?

  • Lifts involving 2 or more hoists or more than 75% of the rated load capacity precluded with a JSA?

Material storage, handling and barricading

  • Is material and equipment stored in racks, stacked appropriately, or secured so as not to shift, move or fall?

  • Are trucks and trailers secure from moving when loading and unloading? Are dock plates used?

  • Are combustibles improperly stored near exits or aisleways that lead to exits potentially causing entrapment if a fire/spill were to occur?

  • Are proper lifting techniques used when lifting material - both manual and mechanical)?

  • Are items stored on shelves properly to prevent potential injury or property damage ( heavier items on bottom)

  • Are storage shelves capable of withstanding loads? Do shelving have load rating showing on them?

  • Are areas posing dangers barricaded properly (all sides and at 36 in high) with red barricade tape?

  • Are temporary work areas or temporary storage areas barricaded properly (all sides and at 36 inches high) with yellow caution tape?

  • Are barricades tagged on all sides noted with who erected the barricade, what the hazard or dangers are and contact info?

Tools and Machinery

  • Are all hand tools in good condition and not broken, bent, cracked, disfigured, or in a condition that reduces the integrity of the tool being inspected?

  • Are tool handles tightly wedged into the heads of all tools?

  • Are tools stored in a dry location and secured where no unauthorized persons can get to it?

  • Are tool cutting edges kept sharp as to reduce possibility of binding or skipping?

  • Are air hoses and fittings in good condition? No cracks, leaks, or brittle.

  • Are grinders, saws, and similar equipment provided with appropriate safety guards?

  • Are portable circular saws equipped with guards above and below the shoe? Are operators wedging guards up to bypass them?

  • Are all cord connected, electrically operated tool of the approved double insulated type?

  • Are fans below 7 ft properly guarded with openings of 1/2 inch or less?

  • Are corded, electric power tools used with GFCI circuits?

  • On abrasive wheel equipment grinders, is the work rest used and kept adjusted to within 1/8 in (0.3175 cm) of the wheel? Is the tongue on the top side of grinder used and kept adjusted to within 1/4 in (0.6350 cm) of the wheel?

  • Are bench and pedestal grinders permanently or securely affixed?

  • Are all pulleys and belts with 7 ft of the floor or at working level, properly guarded?

  • Are all equipment emergency stops colored red?

  • Are foot controls for equipments properly secured and protected to prevent accidental activation from objects?

  • Are all moving chains, gears, flywheels, sprockets, reciprocating parts, etc properly guarded?

  • Are splash guards mounted on machinery that use coolant to protect coolant from reaching employees?

  • Are machines marked/signed for potential hazards (pinch points, crush hazards, etc)

  • Are light curtains and other automated safety cutoff switches in place in and around machinery to prevent operator and other employees from exposing themselves to hazards in the machine.

  • If compressed air is used for cleaning, is it controlled properly to less than 30 psi? Is proper PPE used.

  • Are drills, lathes, presses, jigsaws, etc. have the proper shields, collars, and guarding at the point of operation?

Hot work

  • Is there a designated are for Hotwork?

  • Do employees wear safety glasses and appropriate face shield when doing any Hotwork (cutting or grinding)?

  • If outside designated hotwork areas, are Hotwork permits being used? Are they being turned into HSE department regularly for review and filing (current year only).

  • Are gauges and rigs at portable Welding stations broken down and valves covered at the end of day.

  • Are fuel/oxygen bottles segregated from each other and secured properly (chains or ropes must be 2/3 of the way up) ? Are all manufacturers labels present?

  • Is proper PPE being used for hot work - welding gloves, sleeves, tinted lenses, etc?

  • Is weld flash shielded (curtains or barriers) from other work areas and aisleways where other employees passing are exposed?
