Daily Project Information

  • Is there a Pre-Shift stretching and in practice? Are stretches led by foreman?

  • Has the steel erection Foreman reviewed today work, challenges and safety procedures in the "daily huddle"? Was the huddle signed by the crew?

  • Was a toolbox completed this week?

  • When was the toolbox completed?

  • Is there adequate First Aid kit(s) and supplies available?

  • Is there a certified CPR/First Aid person on site?

General Job Site Safety

  • Lack of/or inadequate amenities? (port-o-johns, wash area, break area, etc.)

  • Housekeeping

  • Are there adequate fire extinguishers and are they in good condition?

  • Is the annual inspection current? Are monthly inspections being performed / documented? No more than 100' to the closest extinguisher? Used units are immediately from service and replaced?

  • Power lines are more than 20 feet away from the point of any work to be performed? (applies to cranes, forklifts, aerial lifts, etc.)

  • All moving equipment is traveling at safe speeds? (no faster than a brisk walk)

  • Adequate drinking water is supplied for workers?

Material Handling

  • Was the PowerLift method used? We're two men used to lift the object? Was there mechanical assist needed?

  • Ask for help, breakup load, reduce weight or use carts or proper equipment when necessary
    Lifting: legs bent, wide stance, chin up, back straight, and lift with your legs, not your back. Use a power-lift stance.
    Lifting/Carrying: Getting proper leverage for use of arms or legs, not back, for pulling and not trying to lift or pull from extended reach. Push rather than pull whenever possible, turn with whole body not waist when carrying load, use good posture; takes regular breaks when kneeling or performing repetitive tasks or for prolonged tasks (such as standing at a workbench, squatting, or holding an item for a long period of time).

  • Hands free of pinch points

Personal Protective Equipment

  • Employees have safety vest, high vis, hard hats, safety glasses, and sturdy boots

  • Ear Plugs / dust masks available/ gloves being used when needed?

  • All welders are wearing a protective welding hood with hard hat while welding?

  • Workers grinding, cutting, or performing any work that does, or may produce flying objects and particles are wearing safety glasses and a face shield?

  • All suppliers and sub contractors involved are in compliance with PPE requirements

Fall Protection

  • Fall exposures of 6 feet or more? (including pits and trenches), employees are protected?

  • Rebar caps in use?

  • 100% tie off in aerial lifts at all elevations (with retractable)? No standing on mid-rail or top rail?

  • Use of correct anchor point in aerial lifts?

  • Personal fall arrest equipment adjusted and worn properly? (chest strap, leg straps, d ring located between shoulder blades)

  • Retractables connected to adequate anchor points?

  • Perimeter (cable) guardrails installed and adjusted properly? (must be a minimum of 39" above walking surface with 200 lbs. of downward force applied and no more than 3'' of deflection.) Wire rope clips have U-bolt over dead end of wire-live end rest in saddle?<br><br>

Steel Erection

  • Bolt-up procedures being followed?

  • All columns are supported by a minimum of four anchor rods, tightened prior to the release of hoisting lines? Structural stability is maintained at all times? (columns are braced in a minimum of two directions within a reasonable time)? Minimum of two bolts per connection during erection? All bolts are installed and torqued prior to loads being applied to the structure? (mezzanine pours, rtu placement, etc.)? Joists are connected at both ends prior to releasing hoisting line? Joist Panels are connected at all four corners before releasing hoisting lines? Where X bridging is required it is installed between joists prior to release of hoisting lines? No personnel are on top of joists without all bridging installed?

  • Safe storage of deck bundles?

  • Minimum of one row of bridging is installed prior to placement of deck bundles? Deck bundles placed on a minimum of three joists? Deck bundles placed within one foot of the bearing end of the joist?

Crane and Hoisting Operations

  • Safety crane operations?

  • Daily inspections documented? All safety devices on I the crane are operational?

  • Good conditions and acceptable wind speed?

  • Swing radius barricade? Crane located on firm, graded, level ground surface for operation? Routes of suspended loads do not pass over workers? No work is performed when wind speeds exceed the manufacturers limitations of the crane?

  • All rigging from a forklift is performed with a hook or hoisting attachment? (no rigging on the forks)

  • Forklift is inspected and being used properly?

  • Forklift operator is wearing a seat belt? Forklift loads are kept close to the ground when traveling? Forks are placed on the ground when forklift is stopped? Daily forklift inspections are completed and there is proof?

  • Do operators have proof of training?

  • Operator of hoisting equipment does not leave the operators position while a load is suspended?

  • Taglines, lifting capacity, tags, and rigging?

  • Tag lines used for controlling loads? Lifting capacity is clearly displayed on all hoisting attachments? Tags are installed on all rigging equipment, and are readable? Has rigging equipment been inspected? Is it in safe, usable condition?

Scissor Lift / Boom Lift

  • Is operator trained?

  • Inspection has been performed prior to use and controls are functional?

Decking and Controlled Decking Zone

  • Perimeter warning lines erected? (15 feet from edge, line must have a breaking strength of 200 lbs.)

  • All workers in CDZ tied off 100%? Roof and floor openings covered/labeled?

  • Retractables connected to adequate anchor points?

  • Loose metal decking is secured against accidental displacement at the end of the shift?

  • Area below decking operations is barricaded, or warning signs and a safety monitor are present?

Ladders

  • Ladder usage is safe?

  • Ladder type is correct for the task? Extension ladder extends a minimum of 3ft above upper surface? A-Frame ladders fully open and braces locked?

Welding, Tools, and Electrical Safety

  • No damage or hazardous conditions with the welding operations?

  • No damage or repairs in welding lead 10' from stinger? Welding ground cable connected? Location of welding leads and extension cables are safe from accidental displacement or damage by personnel or equipment? Fully charged fire extinguisher available?

  • GFCIs are being used?

  • Extension cords have no repairs in the cable, ground prong is present, and are a minimum of 12 gauge wire?

  • Condition of tools?

  • All guards are in place on tools, electrical cords of all tools are serviceable and gave no repairs or missing ground prongs?
