  • 1.Is the access to & from the site/ roof suitable?

  • 2.I have checked & the site is clear of hazardous substances ?

  • 3. Is the site clean of excess debris to ensure safe work area ?

  • 4. Can waste material be appropriately isolated for safe removal ?

  • 5. Do you have your safety warning signs in place ?

  • 6. I acknowledge no other trade will be allowed to work beneath you

  • 7. Are you & your employees wearing appropriate clothing (PPE) ?

  • 8. Are ladders properly set up and secured ?

  • 9. Is the site clear of any overhead electrical hazards ?

  • 10. Electrical equipment has been checked & tagged ?

  • 11. Can the job installed safely with out safety rail. All gutters levels are below 3 meters. ?( if not go to question 12)

  • 12. Can the job be installed safely with no scaffold. All gutter levels below 3 meters ( if yes go to question 13; if no go to question 15)

  • 13. Has the scaffold been satisfactorily provided to your requirements ?

  • 14. Scaffold has not been removed and is signed and tagged ?

  • 15. (2 storey) stairwell void is protected

  • 16. Are the weather conditions suitable for work on the roof ?

  • 17. Have you & your staff received appropriate induction & safety awareness training? ( also all hold blue/white card)

  • 18. Has designated no-go zones been cordoned off, EG beneath elevator hoist ?

  • 19. Are you satisfied with the safety of this job and can you commence work ?

  • As part of regulation 3.1, I the tile fixing contractor being the person having control of the workplace have identified each potential hazard to which a person at the workplace is likely to be exposed; have assessed the risk of injury or harm to a person resulting from each hazard identified and have considered the means by which risks can be reduced. I only commenced tile fixing when I was satisfied that all the requirements as per WA occupational safety & health act 1984 and the uniform code of safe work practices for the roof tiles industry had been met, including and not limited or the above checked items.
