Are workers free from exposure and/or not exposing others to harmful or hazardous substances? (Silica, Lead, Flammable gases)

Housekeeping

Are work areas kept clean and orderly?

Are break areas kept clean and orderly?

Are storage areas kept clean and orderly?

Is there adequate lighting for the tasks?

Is the job site free of slips, trips and fall hazards?

Are nails removed from scrap lumber?

Is trash picked up and put in proper receptacles?

Is sharp metal scrap contained in area by container / trash can or other to prevent anyone walking from being cut by a sharp edge?

Occupational Health and Environmental Controls - Subpart D

Are chemicals stored in a safe location? (flammables away from ignition sources, spills would not enter storm water drains, etc.)

Is there spill containment equipment on site?

Is there sufficient ventilation from potential fumes?

Are containers suitable for the products in it?

Are all containers properly labeled?

Personal Protective Equipment (PPE) - Subpart E

Are all employees following company PPE requirements? (Ex. Hard hats, safety glasses, high vis, work boots, gloves)

If needed based on task, is specialty PPE being used properly? (Face shields, hearing protections, gloves, etc.)

Is all PPE in use safe and free from recognized hazards?

Is extra PPE available as needed?

Fire Protection - Subpart F

Are there Fire Extinguishers on site? (adequate number, type, maintained, inspected)

Are all hot work activities being managed properly to prevent fires? This includes fire extinguishers, hot work permits, and fire watches. (If applicable)

Are LP-Gas tanks in use?

Are compressed gas cylinders being used properly, including: storage, proper separation, use?

Are fuel tanks properly protected from vehicular damage?

Are there fuel tanks on site?

Are fuel tanks protected from leaking? (Double wall tank or containment tank)

Are fuel tanks properly protected from vehicular damage?

Is there a fire extinguisher located no closer than 25' but no more than 75' from fueling areas?

Signs, Signals, and Barricades - Subpart G

Is work being performed around traffic?

Is there a Traffic Management Plan (TMP) in place for work being conducted in the roadway?

Are flaggers properly trained?

Do controls for TMP function properly and are free from damage? (Physical barriers, bollards, speed limits, flashing lights, spotters, etc.)

Materials Handling, Storage, Use, and Disposal - Subpart H

Is rigging and signaling being used?

Do riggers and signal persons have documentation of qualifications?

Are all pieces of rigging inspected prior to use for damage?

Are all pieces of rigging safe, properly labeled, and undamaged? (ex. wire ropes, shackles, slings, chains, spreader bars, hooks)

Are loads being rigged properly using the correct type and configuration of rigging for the load?

Tools - Hand and Power - Subpart I

Are power and hand tools in use?

Are all tools in proper working order and without damage?

Are electrical cords free from any and all damage?

Is GFCI protection being used on all temp power?

Are outlets and circuits free from overloads?

Are all live parts protected from accidental contact?

Are electrical cords protected from being placed on sharp ground or hung by conductive metal?

Are electrical cords and equipment in a safe location, not near flammables, or lying in water?

Is electrical work being conducted on de energized locked and tagged equipment? (LOTO)

Are receptacles installed in a wet or damp location designed for the location?

Scaffolds - Subpart L

Are scaffolds in use?

Was the scaffold assembled under the supervision of a "Competent Person"?

Is the scaffolding properly tagged?

Is the competent person performing daily scaffold inspections?

Are scaffolds free from damage and are they complete?

If electrical power lines are near the scaffold, are necessary precautions being taken?

Are all scaffold components compatible?

Are all cross braces properly installed?

Are all stairs on site safe and free of recognized hazards?

Are all stairs on site properly constructed? (Metal pans stairs filled, hand rails, proper rise to run, Properly secured, and slip resistant)

Are ladders in use?

Are all ladders on site safe and free from damage?

Are all ladders on site being used in a safe manner that is consistent for the use they were intended?

Are all ladders properly secured?

Do all ladders meet minimum OSHA requirements? (Proper weight rating for the work being done)

Are all cranes on site free from damage and recognized hazards?

Are all loads being lifted in a safe manner? Avoid loads lifted over persons, close proximity to obstacles incl. overhead power lines?
