Construction

ADMINISTRATIVE

  • Is there a qualified person on site?

  • Are all signs and closures set up properly?

  • Is there a site specific accident prevention plan on site?

  • Have all the confined spaces been identifed and employees aware of KCRC proceedure?

INSPECTIONS

  • Is there proper guarding on all equipment, pinch points, sharp objects, and moving parts?

  • Have inspections been done on lifting chains, cable, and straps?

  • Do all the lifting chains, cables, straps have a MIOSHA weight tag on them?

  • Does all requierd equipment have slow moving vehicle signs on them?

  • Are all power tools been inspected for proper guarding and in good working order?

  • All loads checked to be secure during transport and before releasing tie downs?

OPERATIONAL

  • Are all Materials, scraps, or debris piled and stored as to not create a hazard?

  • Are excavations not readily visible protected by barricades or fencing?

  • Is all equipment using strobe lights?

  • Are Truck Drivers and Equipment Operators wearing seatbelts if there is a ROPS system in place?

  • Are all wheeled trucks and equipment that are parked on an incline blocked or chocked?

  • Are all trucks and equipment operating in a safe manor (no horse play, working on stable surface)?

  • Have overhead power lines been identified?

  • Are overhead power lines coned off at a distance of 10' when using derricks or cranes?

  • Are we maintaining a 10' clearance from all power lines?

  • Are all employees staying away form elevated load over head height?

  • Has Miss dig been called?

  • Is there a copy of positive response on site?

  • Have utilities been located by hand within 4' of any utility before digging with equipment?

  • Is the area free of any trip hazards?

  • Is everyone wearing a safety vest?

  • Is everyone wearing safety glasses where needed?

  • Is everyone wearing hardhats and appropriate areas?

  • Does everyone have of safety toe boots?

  • Are all tools and equipment being used for their intended purpose?

  • Are there designated area's for foot traffic and equipment traffic?

  • Is the fuel tank grounded?

  • Are all containers properly labeled?

  • Are Tourche lines free of bulges and cracks, and shut Off and lines cleared at end of shift?

FALL PROTECTION

  • is there proper fall protection in place (railing, Harnesses, holes covered and identified)?

  • Are all fall arrest systems inspected before each use?

  • Are all fall arrest systems adjusted and worn correctly? ( 2 fingers beneath the strap, D-ring in center of back, chest and legs adjusted properly)

  • Is there a rescue plan in place that assures rescue within 15 minutes?

  • Are holes greater than 2" in dimension protected by covers that will support twice the weight of employees, secured, and marked with words "hole" or "cover"?

  • Can all railings resist a load of 200 lb when applied 2" from top, are 42" +/- 3" high, mid rail at 21" high from working surface platform, and toe board to prevent objects from falling below?
