ADMINISTRATIVE

  • Is there a qualified person on site?

  • Are the road closure or signs set up properly?

INSPECTIONS

  • For excavator or Gradeall is there a copy of annual equipment inspection on site?

  • Have inspections been done on lifting chains, cable, and straps?

  • Do all the lifting chains, cables, straps have a MIOSHA weight tag on them?

  • Has a daily inspection been done on trucks and each piece of equipment?

  • Is there proper guarding on all equipment, pinch points, sharp objects, and moving parts?

  • Are all power tools in good working order?

  • Are loads checed to be secure while in transport and before releasing tie downs?

OPERATIONAL

  • Are Truck Drivers and Equipment Operators wearing seatbelts if there is a ROPS system in place?

  • Are all trucks and equipment operating in a safe manor (no horse play, working on stable surface)?

  • Are all trucks that are parked on an incline block or chocked?

  • Are all Materials, scraps, or debris piled and stored as to not create a hazard?

  • Is the area free of any trip hazards?

  • Have overhead power lines been identified?

  • Are overhead power lines coned off at a distance of 10' when using derricks or cranes?

  • Are we maintaining a 10' clearance from all power lines?

  • Are all employees staying away form elevated load over head height?

  • Is everyone wearing a safety vest?

  • Is everyone wearing hardhats and appropriate areas?

  • Is everyone wearing safety glasses where needed?

  • Does everyone have safety toed shoes?

  • Has Miss dig been called?

  • Is there a copy of positve response on site?

  • Have all utilites been located by hand before using equipment, if we are diging within 4' of them?

  • Are there designated area's for foot traffic and equipment traffic?

  • Are tools being used for their intended purpose?

  • Are GCFI's being used?

FALL PROTECTION

  • Is there proper fall protection in place (railing, hole covers, fall arrest systems)?

  • All fall arrest systems inspected before each use?

  • Are all fall arrest systems adjusted and worn correctly?( 2 fingers beneath strap, D-ring in center of back, chest and leg straps adjusted properly)

  • Is there a rescue plan in place that assures rescue within 15 Minutes?

  • Holes greater than 2" in deminsion protected by covers that will support twice the weight of employees, secured, and marked with words "hole" or "cover"?

  • Can all railings resist a load of 200 lb when applied 2" from top, are 42" +/- 3" high, mid rail at 21" high from working surface platform, and toe board to prevent objects from falling below?

  • Are all mirrors clean, adjusted properly, and free of visual distortion?

  • Is the boom and Stick free of cracks, bends, or unusual ware?

  • Are the bucket hooks secured properly, free of cracks, and the hooks have less than 10 degree twist and 15 degree spread?

  • Are all track idlers, sprockets, adjusters, and rollers in good working order?

  • Are all pins and bushings secured and free of unusual ware?
