1. Job Set Up

  • Is there Keller signage on site?

4. Fall Protection

  • Restraint is used such as retractable lifelines before fall protection is used?

5. Ladders

  • 4:1 vertical to horizontal ratio for extension ladder set up?

  • Unsafe or damaged ladders, cracked rails, loose feet, missing rivets?

  • Unsafe positioning of ladders? Extends 36" above exit level?

  • Ladder not properly secured?

  • Ladder unsuitable for job? (e.g. metal ladder used for electrical work)

6. Scaffolding

  • Safety factor of "4" is met?

  • Scaffold grade material is used for all working surfaces, decked from front to back 18" min.

  • Scaffold is on a solid foundation and mud sills if needed are secured?

  • Guard rails are erected on all scaffold over 10' above a lower level, are toe boards required?

  • Wheels are locked/ chocked to avoid movement?

  • Scaffolding is secure, pinned together and properly braced and to the building as required?

7. Guarding

  • Are all warning lines installed and flagged for visibility?

  • Elevation changes of 19" or more has an access route, ladder, ramp etc. ( no jumping up and down)?

  • Is all rebar and plumbing protected?

  • Stairs, temp rails needed at all stairs with a 30" rise or with 4 or more threads?

  • Floor and wall openings, covers secured and labeled "HOLE"?

  • Guard rails when more than 6' above a lower level?

8. Electrical

  • Are overhead or underground power lines a concern?

  • Temporary lighting is hung correctly, not by the cord and all bulbs are protected?

  • Cords are hard or extra hard usage rated? S,,ST, SO, STO etc. 12gage or larger.

  • GFCI - testing/ taping is in place? All tools are grounded or double insulated?

  • Cords are routed to avoid damage to them, prefrebly overhead

  • All access holes to the power panel are plugged or used?

  • All power panels are covered with proper cover, not cardboard?

9. Housekeeping

  • Material is organized and stacked safely and protected from damage and weather?

  • Tools are organized and stored to avoid damage?

  • Work areas and walkways are free of clutter and debris?

  • Container for scrap is provided?

  • All potential hazards are addressed such as nails removed from lumber, sharp items and rough edges?

10. Work Environment

  • Lack of/or inadequate amenities? (toilets, wash areas, lunch rooms, etc)

  • Does the site prep create a hazard due to inadequate backfill or rough grade?

  • Does scheduling cause hazards for Keller or our subs? Are we working over others or vice versa?

11. Jobsite Security

  • Is the site fenced in?

  • Are all warning signs in place?

  • Are security lights in place?

  • Is access barricaded when Keller is not present?

12. Welding and cutting

  • Is a hot work permit used?

  • Welding is performed by a qualified person?

  • Compressed gas secured in a upright position?

  • Fire extinguisher available?

  • Area clear of flammables?

  • Equipment being used as designed, (friction lighter)

13. Fire protection

  • All flammables such as fuel are stored away from buildings, ignition sources, and water?

  • Storage of all flammable material such as lumber and paper products is safe?

  • Fire extinguishers are provided? One for every 3000 sq ft or 100 feet of travel?

14. Aerial lifts and Scissors lifts

  • Equipment is being used as designed?

  • All safety measures are in place such as gates closed, fall restraint is worn as required?

  • Pre-shift inspections have been performed?

  • Operated by a certified/ trained individual?

15. Cranes

  • Operator is trained and certified?

  • Are overhead power lines a hazard?

  • Hand signals and warning labels are posted?

  • Swing radius is protected?

  • All operating features are in working order and available, load capacity, boom angle, LMI, anti- two block etc.

  • All guards are in place?

  • Wire rope and rigging is used according to standards?

  • Windows are free of cracks and clean?

  • All lifting components, chains, slings, hooks etc. have been inspected before use and removed if defective?

  • Daily and annual inspections are current?

16. Equipment-forklifts-skid steer-auger truck- backhoe etc.

  • Used within design and load limits for intended purpose?

  • Potential hazards have been identified?

  • Daily inspections have been completed?

  • Trained/certified operator?

17. Trenching and excavating
