  • Is scaffold fully level and square with cross bracing?

  • Is proper access provided for scaffolds?

  • Is the scaffold <14" from the working surface?

  • Is fall protection provided for scaffolds at 10' and up?

  • Do all scaffolds using guard rails have toe boards?

Hoists/Cranes/Derricks

  • Are guards over moving parts in place?

  • Are cables and sheaves inspected regularly?

  • Is equipment firmly supported?

  • Are outriggers used if needed?

  • Are power lines inactive, grounded, removed, or at a safe distance?

  • Are proper loading and lifting limits being observed?

  • Are signalmen used where needed?

  • Is equipment properly lubricated and maintained?

  • Are inspection and maintenance logs maintained?

Excavations

  • Are excavations more than 5' deep properly sloped, shored, or shielded?

  • Are adjacent structures properly braced?

  • Is material stored at least 2' from edge of excavation?

  • Is equipment operated a safe distance from the edge of the excavation?

  • Are ladders provided so no more than 25' of travel is required to exit the excavation?

  • Are equipment ramps adequate?

  • Have all underground utility lines been ID'd and marked?

  • Is a competent person on site at all times?

Welding and Cutting

  • Are operators qualified?

  • Are screens or shields being used?
