Section 1

  • A) What are the critical characteristics of this vehicle, which makes it “High Risk Classified”?

  • * High Center of Gravity - COG is 48” above vehicle frame/chassis “and/or”
    * Potential for load shift (fluid, material, etc….) “and/or”
    * Towing of trailer with high center of gravity load “and/or”
    * Vehicle and/or load height exceeds 14 feet or Greater ( OPL Required )

  • B) How were you qualified to operate this High Risk Vehicle?

  • * Current and properly licensed (CDL, Endorsements, Medical Card)
    * Skills Assessment completed and authorized by company SME
    * Completed Defensive Drivers Training, including a Commentary Drive

  • C) What Pre-Trip Activities, were performed, prior to departing your initial location?

  • * 360 degree walk-around is performed to clear vehicle path of obstructions
    * DVIR (daily vehicle inspection report) completed to ensure vehicle is safe to operate ( DOT or similar)
    * All loose items secured inside vehicle and outside of the vehicle
    * Route identified, approved and maps inserted, if needed
    * Possess a documented Journey Management Plan form
    * Distractions set aside (electronic devices stowed, radio tuned, etc….)
    * Fit for Duty ( Proper rest period, Alert, Performance not effected by illness or medications)
    * Signed acknowledgement for completing journey plan safely and Incident Free.

  • D) What measures do you take to ensure your driving behaviors are appropriate for the weather/visibility conditions?

  • * Get weather information for your destination and waypoints.
    * Postpone journey until more favorable conditions are present.
    * Keep safe distance from vehicle ahead. (4 second rule, etc.).
    * Reduce speed to prevent hydroplaning, skidding, poor traction when braking, or other hazardous condition.
    * Know the posted speed limits for area ( paved versus non-paved )
    * Verify vehicle warning systems and devices fully functional

  • E) During Trip - Just in Time (JIT) Journey Management<br>Due to the ever-changing nature of our lease roads, journey management in the field must be performed in an on-the-spot manner requiring constant awareness of conditions and immediate mitigation steps to be taken:

  • What key road criteria need assessed for safely completing the driving journey?
    * Road integrity (decomposition, potholes, washouts, rutting, etc…)
    * Road slope (appropriate for vehicle type & load, appropriate gear, appropriate speed)
    * Road width in relation to my vehicles (turn radius, size, height, cargo)
    * Eye lead distance obstructions (blind corners, blind landing area, oncoming traffic)

    When any of the above criteria are determined unacceptable for vehicle type and load:
    * Alternate route taken
    * Stop Work Authority shall be utilized
    * Supervisor shall be consulted regarding mitigation steps, if needed

  • F) When parking your vehicle, what is the preferred vehicle position?

  • * Answers must be specific - The intent of First Move Forward (FMF) is to prevent injuries and damage to property that could occur due to blind spots and undetected movement of personnel and equipment that result from unsafe vehicle positioning/backing practice.

  • G) When backing is unavoidable, what actions shall be completed, prior to positioning/backing your vehicle?

  • * Driver Responsibilities:
    o The driver shall complete a JSA (Job Safety Analysis) or entry on existing JSA, prior to staging equipment on location
    o The driver shall make a complete 360-degree walk around of the vehicle to determine if any obstructions and potential hazards are present in the proposed path of travel.
    o If using a spotter, inform spotter of hazards and always maintain visual contact. Lose sight of spotter, stop vehicle immediately.
    o If no spotter is available, cones are acceptable. Same visual rules apply.
    o The driver shall also check to ensure overhead and side clearances are adequate.

    What other situation, would a spotter or cone be needed?
    * When vehicles must negotiate forward turns with restrictive side clearances and where height clearances are uncertain, a spotter or traffic cones shall also be used.

  • H) What potential site hazards must be assessed prior to stopping or parking your vehicle?

  • * Site congestion ( people ,other vehicles and process equipment )
    * Drop offs, soft shoulders/loose dirt
    * Incline/decline
    * Weather, muddy or wet conditions, rutted path
    * Fixed objects
    * Objects obscured from view (hidden by vegetation, etc.)
    * Overhead power lines

  • I) What steps must be followed, when parking your vehicle on an incline or decline?

  • * The front wheels shall be properly turned to the curb or bank
    * The parking brake shall be engaged.
    * When engine is turned off, vehicle must be in gear (manual transmission) or Park (automatic transmission) * Neutral if company approved SOP
    * Shall properly chock their wheels.

  • J) What steps will you commit to ALWAYS complete before exiting the vehicle?

  • * The engine shall be turned off
    * The parking brake set
    * The transmission placed in park (or in gear for manual transmissions)
    * Remove keys from ignition
    * And all doors closed
    * If vehicle will be left running for use of PTO or Auxiliary equipment, a company approved SOP is required.

  • K) What steps will you commit to ALWAYS complete when stopping your vehicle while remaining in the vehicle to perform some other task?

  • * Always set parking brake.
    * Always place gearshift in park if automatic transmission or if manual transmission placed in neutral and set brakes.
    * Never leave any vehicle in gear with foot on brake.

  • L) What measures do you take to ensure your safety while moving through intersections (public and/or lease)?

  • * On rural, small or dirt roads with unobstructed visual field; scan for other vehicles, slow or stop as appropriate and proceed when safe to do so.
    * On public roads, obey all traffic laws, signs, lights and warnings.
    * Do not assume that other drivers will do the right thing, use defensive driving principles.
    * When using lease roads that intersect public roads; stop, yield right of way, ensure path is clear and then proceed through the intersection.
    * Recognize high rate of speed of cross traffic.

  • M) What steps do you take to prevent vehicle rollover?

  • * Must be familiar with the size and type of vehicle/equipment you are operating and be experienced in its operation.
    * Assess all roads/areas being traveled.
    * Must know the turning radius capability of the vehicle/equipment you are operating.
    * When unsure, do not put your vehicle/equipment in a hazardous position by using trial-and-error maneuvers in an attempt to complete a tight turn.
    * Get the Big Picture by using mirrors, spotters and walk-arounds.
    * Never exceed the speed that is safe for the set of conditions that are present.
    * If you “Think” you can safely make route, use Stop Work Authority and take mitigating actions.

  • Awareness Only: How do you go about reporting unsafe or damaged roads?<br> <br>* Report to Supervisor, HES or Motor Vehicle Safety representative.<br>* Immediately barricade/block roads that could lead to IDLH situations and summon assistance to ensure road cannot be used until necessary repairs are made.
