1- Epix work sheet completed, RAMS book present & referred to?

2 - Is the work equipment list present, completed, appropriate used?

2.1 - Is the work equipment sheet on the server?

2.2 - Vibration and noise controls held, understood and adhered to?

3 - Tools PAT tested and / or calibrated within the last 12months?

3.1 - Do the PAT test labels match the certificate?

3.2 - Is the PAT test certificate on the server?

4.1- Is the ladder inspection sheet on the server?

4.2 - Working At Height - safe, edge protection in place?

5 - Is the operative clean shaven with tested/suitable RPE in place?

6 - Are suitable controls in place for asbestos? Is the asbestos emergency kit present?

6.1 - Has the engineer read the asbestos survey report and can they accurately identify where it is located?

7 - Has the engineer suitably controlled all risks from the task / workplace? Are barriers present?

7.1 - Is the engineer working safely?

8 - Does the operative have current company ID card with him?

9 - Is the engineer suitably competent for the works being undertaken?

9.1 - Is the competency assessment form on the server, in date?

10 - Is the first aid kit present and in date?

10.1 - Is the engineer well presented and courteous? Is the correct PPE being worn incl. task specific PPE, such as hard hat, gloves, RPE etc.

11 - Tower scaffold, PASMA, used appropriately?

12 - MEWP, IPAF, 6 month check, harness?

13 - Any reports of Near Miss submitted by engineer?
