Documentation

  • Is there a Project Management Plan (PMP) in place and have all staff read, understood and signed the PMP?<br>

  • Are there adequate spare documents such as JSAs / Risk assessments / RISK cards / lift plans / tool box talk records for the duration of the works?

  • Are tool box talks being carried out on a daily basis to ensure daily risks and activities are communicated to all.

  • Is there a first aid box on site adequate for the number of personnel on site and is the contents in date (complete a 1st aid checklist)?

  • Is there an eye wash station on site adequate for the number of personnel on site and are the contents in date?

  • Are there adequate first aiders per shift and are these first aiders identified and known to all on site? (one first aider required per shift).

  • Is there a stretcher available?

  • Are there any additional medical facilities available?

Fire

  • Is there a flammable cabinet on site?

  • Is this sufficient to store all flammable substances on site?

  • Has a fire risk assessment been completed of the site and have the findings been communicated to all on site & a site plan displayed?

  • Are there adequate fire extinguishers on site and are these in date for testing (needs to be completed annually).

  • Have personnel been trained in the use of a fire extinguisher?

  • Are fire drills carried out regularly?

  • Is the muster point clearly marked and are all personnel aware of it?

  • Is a record of who is on site maintained?

  • Do all substances on site that have a hazard label on them have a COSHH assessment and Material Safety Data sheet?

  • Have these been communicated to site personnel?

Storage of hazardous substances, spill & waste management

  • Are there adequate absorbent pads / granules available for oil spills?

  • Are there adequate drip trays on site for refuelling / oil change activities?

  • Are there adequate bunded facilities for the storage of oils / fuels etc to enable bunding to 110% of the volume of the largest container on the bund?

  • Are the bunds protected from water ingress?

  • Is the site layout well planned so that hazardous substance storage is out of the way of traffic movements?

  • Are there adequate bins on site to enable waste to be streamed and contained securely?

Lifting

  • Are there blank lift plans available within the PMP?

  • Do the lift certificates contained in the PMP match with the actual lifting equipment on site?

  • Is all lifting equipment and ladders on site in test and marked with the current safe working colour?

  • Is there a safe working colour poster displayed on site?

Working at Height

  • Are there adequate lanyards and harnesses on site for the activities being completed on site?

  • Is all working at height PPE in test and marked with the appropriate safe working colour?

  • Are all personnel trained in working at height and aware of the risks and how to wear the PPE correctly?

Working over water

  • Are you able to comply with the control measures detailed in the relevant boat transfer risk assessment?

  • Are there adequate life jackets available to ensure that all personnel have one?

  • Are there adequate buoyancy aids on board to ensure that all personnel have them that need them?

Jack Up / Vessel

  • Is an induction carried out specific to the vessel? If so have all LDD personnel been inducted?

  • Are all handrails on the vessel / drill rig suitable and sufficient to ensure the safety of those working over water?

  • Are there adequate emergency response plans for man overboard scenarios and have these been communicated to all LDD staff on site?

  • Are regular man over board drills carried out?

  • Are life rings positioned in convenient locations?

  • Is there a MAYDAY procedure poster displayed?

  • Is the life raft capacity sufficient for the number of personnel on board?

  • Are there enough transfer suits for the number of personnel transferring at any one time?

Communication

  • Are there RISK cards available on site?

  • Are there incident reporting forms available on site?

  • Are there adequate provisions to communicate the emergency services and the office as necessary on all shifts?
