Lifting Accessories

  • Are routine (daily/weekly) checks on the lifting equipment, including the crane carried out and recorded?

  • What lifting accessory is being used at the time of the audit?

  • What is the serial number?

  • Is the accessory within the certification date?

Competencies

  • Is there suitable numbers of Appointed Persons available and authorised for lifting activities on site?

  • Does the crane operator have certificate/card of competence?

  • Does the rigger/slinger have certificate/card of competence?

Documentation

  • Are excavators used for lifting? If so, is it covered by the lift plan?

  • Is there a lift plan for the lifting operations being carried out and approved by Appointed Person?

  • Has the lift plan been briefed to those involved in the lift/s?

Control of area during lifting operations

  • Was the lifting zone secure and controlled?

  • Was the lift carried out in a safe and controlled manner?

  • Is access and egress to lifting equipment available and suitable? have suitable set down areas been identified?

  • Are safe material unloading/loading arrangement is place to reduce the need to climb onto flatbed lorries or their loads?

  • Have all loads been secured properly? When lifting loose loads, bundles of tubes, rebar or timber is the double wrap choke hitched used? If single use slings are used, is it removed/disposed after the lift is completed?
