  • Is there an approved SWMS in place and communicated?

  • Take a photo of the SWMS cover page in use for the activity.

  • Has the Mobile Elevating Work Platform (MEWP) been checked and authorised for use on the site?

  • Take a photo of the current plant inspection sticker.

  • Is there a Daily Pre-Start Inspection Book kept with the MEWP? Has it been filled out by the operator?

  • Take a photo of the daily inspection book

  • Is the MEWP a boom type MEWP? If so secondary protection systems must be in place.

  • What type of secondary protection is fitted?

  • Has the operator been suitably trained and their competence assessed?

  • Check the operator has a current WP ticket.

  • Are barricades required to be erected in accordance with the approved SWMS?

  • Take a photo of the area.

  • If required, does the barricaded area prevent access by other workers?

  • Has the MEWP been set up on firm and level ground?

  • Take a photo of the ground conditions

  • Does the MEWP have outriggers? If so are they fully extended?

  • Is the MEWP operating within the 'zone of influence' of an excavation/trench/open precast concrete cable trench or drain?

  • Does the SWMS show that the MEWP will be operating near overhead powerlines? If so is the operator complying with the requirements of the SWMS?

  • Is the MEWP floor clear of debris, waste, or unnecessary materials and equipment

  • Are there objects in the MEWP that allow workers to increase their height or to sit on (e.g. boxes, chairs)? NB: If so they must be removed immediately.

  • If the MEWP is being operated outside, are the weather conditions suitable for the required operation in accordance with manufacturer's instructions?

  • Has the Safe Working Load (SWL) marked on EWP and SWL of the working platform been exceeded?

  • Does the SWMS require a safety observer or spotter to be available at all times to provide additional guidance when EWP is travelling? If so, is one present?

  • If the MEWP is a boom type, are there fall arrest / restraint systems attached to a suitable anchorage point and worn correctly?

  • Where fall arrest or fall restraint equipment is being used, have the workers been trained in its specific use?

  • Have workers completed working at heights training within the last two years or holders of the ticket but have have operated it several times in the last six months?

  • Is the MEWP inspected by a qualified mechanic / fitter at least every 3 months?

  • Are all tools materials and equipment secured to prevent a dropped object from falling from heights
